Is the diplopia monocular? That is, is it still present when you cover one of your eyes? Or is it only present with both eyes open?

That is always something doctors have to consider: non-organic, non-physiological or functional causes -- which can be either malignering (intentional) or hysteria (subconscious).

You've got enough symptoms where serious organic disease needs to be ruled out via a comprehensive workup.
