Cataracts and astigmatism

My mom just told me that as her cataracts became worse, lights became
elongated.  Her quack told her that cataracts always caused that.  Given
that he apparently botched her surgery (glare, increasing to
near-intolerable levels now even with sunglasses) and didn't notice the
macular bubble, I doubt everything he ever said.

Can cataracts REALLY cause astigmatism?  How?  Since it went away after her
cataract surgery (set by the quack for near vision, she was never consulted)
maybe it's true...

What, or who, is "the quack"?

My mom's long-term 4-appointments/year ophthalmologist who didn't tell her
she could have a choice of near or far vision when she had her cataract
operation, was perhaps one of the last ophthalmologists to still use
stitches, left her eyes increasingly sensitive to light, and ultimately
missed the macular bubble which destroyed the central vision in her GOOD
eye. He took part of her life away. I want the bastard to rot in hell.

Look at it from the other end.  Cataract surgery could remove an
existing astigmatism.  It may be the way he placed the incision, or,
he could have done an LRI (Google is your friend) at the same time to
remove the existing astigmatism.

a wouldn't say that they "always" do that but they commonly do.
cataracts cause glare and can induce astigmatism.

yes.  they alter the refraction of light as it passes through the lens
and thus induce "lenticular" astigmatism.

Astigmatism can come from the cornea or the crystalline lens, and the
two are additive optically, so that depending on the axis of the cyl.
induced by the lens (cataract or not), it can increase the total
astigmatism, decrease it, or just change its axis (orientation).  The
cataract surgery always eliminates the lenticular astigmatism, but the
IOL can have some astigmatism, either on purpose (toric IOLs) or
accidentally (tilted or decentered IOL).  Corneal cyl can be reduced or
eliminated by LRI at the time of the cataract surgery, but the limit
seems to be about 2.5 D.C. of corneal. In all cases, regular astigmatism
can be corrected with glasses, and irregular astigmatism can only be
corrected with RGP contacts or corneal surgery.

Thanks, I guess I'll have to give the bastard one point for accuracy.  She
had no astigmatism before the surgery.

There are several types of cataract and not every person will have the
same experience.

The most common cataract is one in which the center part of the lens
gets denser.  As it progresses, the patient usually has an increase in
myopia and against the rule astigmatism.  I have seen a few  patients
gain 6-8D of myopia and 4D of astigmatism before surgery.
