The lens is too steep, too convex, either front or back or both.
In some cases it may be steeper in the center and flatter at the edges, or
vice versa. That's how we get the contradiction between retinoscopy and
actual acuity.

I'm pretty sure it's not the muscle - the lens just grows that way.
