Is my prescription complete?

I just picked up my glasses prescription today and it looks incomplete. The prescription is as follows:
150 x 117
125 x 082
No other information appears.

All other prescriptions I have gotten include sphere, cyl and axis for each eye. Since my sphere is in the
neighborhood of -6.00 and there is no similar number included in this prescription it raises a question.
These numbers look more like they could be just cyl and axis. Is there some other way of stating a prescription
for single vision glasses such that the prescription stated above seems reasonable?

The Rx above is actually:
1.50 x117 degrees
1.25 x 082 degrees

Yes, the sphere is missing. Maybe because your optometrist doesn't like that
you are not purchasing the lenses from them. Of course, they will tell you
that is was just a mistake, and not intentional.
