My wife (34 years old) has a very painful joint which she has now had for a
year. She has had blood tests, biopsy, ultra sound and x-rays but no
definite diagnosis here in London, UK. She is now in bad pain and is
desperate for a diagnosis so she can be put right. The dr's say they have
never seen such a case of inflammation in a single joint.
All advice appreciated.

I'm not going to be much help, CS, but I'm sure someone else will be along
to help soon.
In the meantime, try posting this on alt.support.arthritis
That's mainly US based, but more posters to give advice, and I know that
some have had ankle problems.

Late answer... I'm just catching up...
My problems began waaay back with my ankles (note, for me it's *both*
which is significant because RA is symmetrical) for about 10 years I
had gradually worsening pain, but just in the ankles.. I had every
treatment and test imaginable and nothing helped... eventually other
joints began to go the same way (most notably my hands) and I started
treatment for RA. However, nothing helps the intense pain in my
ankles.. not very helpful I know, the only advice I can suggest to her
is - don't stand (still) for long periods, keep the ankle raised as
much as possible, strong painkillers and painkilling gel.. oh and
shoes... my point of max pain is right at the bottom of the tendon,
just where normal shoes grip, so I wear only boots and mules..

My O.A. started in my left heel way back. has now got progressively worse
all over so I would say persevere with GP etc there has to be a reason for
the pain.
