I notice that if the pillow I use is too low, the sound of ringing
becomes louder.

I think the above is one of the pieces of information which could help
your doctor diagnose you.  I remember a doctor friend once told me
that when it comes to finding out the cause of an illness, a doctor is
no better than a stock broker.  So while you are asking help from a
doctor, please do help the doctor yourself.  Diagnosis could be a
process of searching in the dark.

There are many causes of T.  Thus while you are having its symptoms
controlled, you owe it to yourself to find out why you got that T.
One of the better ways to go about on this is to give each doctor no
more than 3 chances(i would give only 2), and you tell who is getting
at the 'truth' by what they do for you.  The test of the pudding is in
the eating and eating only!

Most people know their cause/reason of tinnitus but I suppose
some are idiopathic(without any obvious cause) in nature. I disagree
with your doctor friend; most "illnesses/diseases" have a known cause-
a viral conjunctivitis is caused by an adeno-virus and so on.

Even IF a diagnosis, as in tinnitus, is made it still doesn't necessarily
help in finding a cure or treatment. I know the cause of my tinnitus-
it is of no help in finding a cure.

My doctor friend is searching in the dark like many others but it is
the spirit that counts.  It *helps* but is not indispensable that you
should get to know the cause.  Many illnesses are treated without
knowing what caused it.
