Abstract
Objectives: The conventional management of Allergic Fungal Sinusitis
(AFS) after surgery consists of systemic and topical steroids to
immunomodulate the body¡¦s response to fungi. However, there are
significant side effects to prolonged steroid use and there are a
group of patients who are unresponsive to standard treatment. The role
of systemic anti-fungal drugs in AFS is still largely unknown. This
pilot study was carried out to evaluate the effectiveness of
itraconazole, an oral anti-fungal drug in the treatment of refractory
AFS.

Methods: Twenty-four patients with AFS who had had image-guided
surgery and were refractory to prednisone, topical steroid and
Amphotericin B nasal sprays were treated with itraconazole for at
least three months. All patients were evaluated with pre and
post-treatment endoscopic examinations, serum IgE and RSOM-31
questionnaires. Monthly liver function tests were done to monitor for
hepatic side effects of itraconazole.

Results: Nine patients had an improvement in endoscopic stage. Nine
had no difference and five had a worse endoscopic stage after three
months. One patient had to stop treatment due to abnormal liver
function tests. Mean pre-treatment IgE was 722ƒÝg/L and post-treatment
IgE was 700ƒÝg/L. Subjectively, seven patients reported significant
improvement, eight had moderate improvements and eight reported little
or no change. No correlation was noted between the subjective and the
endoscopic findings.

That is really dumb:
It seems they are deliberately trying to
disprove fungus as the menace it is.
First things first: they are working with
allergic reaction to the fungus infection in the
nose. In other words you can have a fungus
infection or not and still, at the same time,
be allergic to the fungus (Candida for example).
Second: Steroids make fungus worse - wa la.
Third: The fact they used steroids makes it a
miracle that anyone improved as far as the
?infection? goes, however that wasnot about
what the test was supposed to be. They
started out wanting to know about the allergy
reaction, and wind up reporting on the
infection: shear insanity. We still donot know
what effect that really, really dumb
concoction of medication has on allergy from
fungus infection - or - uh - what it did for the
allergy (whatever it was) that the fungus - and - or
- fungus infection - uh . . . or - uh - did the
concoction relieve the ALLERGY symptoms
for the patient - and - uh - oh well . . .
Fourth: The fact there was improvement in
spite of the steroids, proves the over-whelming
power of the fungicide against the
fungus infection, which had nothing to do
with the test..
Fifth: Since this was all about allergy
reaction to fungus infection, not fungus infection,
the test makes no sense, and was a complete bust . . .
and i guess we taxpayers paid for this scam.
Those schemers belong in jail!!!!!!!

On the contrary I think it is more evidence that fungal sinusitis is a
substantial menace, and incurable too.

The Huffnagle study I previously posted showed that candida fungus in
the GI tract causes allergy to harmless fungus in the respiratory
tract.  Thus whether the fungus in the respiratory tract is
technically an infection or not is basically a moot point.  Either way
the answer is to take drugs to kill fungus, throughout the body.

I'm with you there.  Steroids compromise long term recovery for the
sake of short term symptom relief.

I agree the study may be flawed in several respects.  Better to use a
placebo control; better to use a newer antifungal - voriconazole, as
opposed to a much older less effective antifungal, itraconazole.

Nevertheless the fact that studies like this are being done at all is
reason to have hope.  If 30 Million americans have the disease,
whoever invents a substantially better antifungal will make money.
