Antibiotic Advice

First, let me say that I am very happy to have found this resource,
and thanks to the doctors here who spend their time investigating
sinusitis. I have a question that I hope Dr. Grossan or one of the
other doctors might be able to answer for me.

I get sinus infections about two to four times a year. They became
more frequent after I had my wisdom teeth removed about four years
ago, and my sinus was perforated (and became infected, cleared by two
rounds of antibiotics). I moved out of a moldy house a year ago, which
has improved my symptoms considerably, but hasn't eliminated them. I
use saline irrigation (I have a Grossan irrigator attachment for my
Water-Pik and a nasal wash bottle), guaifenesin and Rhinocort AQ as
needed, and I drink lots of water and hot tea.

I recently came down with a mild infection, and my doctor put me on
the five-day zithromax (Z-Pak). I have had adverse reactions to most
of the other antibiotics indicated for sinus infection, and zithromax
is one of the only antibiotics I can take. Several days after
completion, I woke up and expelled a small amount of slightly bloody
clear yellow mucus. I told my doctor and he prescribed another round
of Z-Pak. Since I completed that round, I've woken up several mornings
and expelled small amounts of the same stuff. My doctor has prescribed
me another two rounds of Z-Pak, and referred me to get a CT scan of my
sinuses.

My question is this. Given the potential risks of antibiotic therapy,
does it make sense for me to take two more rounds of antibiotic, when
my symptoms are very mild? Is there a better option? Also, how often
should I disinfect my irrigator or nasal wash bottle while I have an
active infection? Every time I use it?

I'm not a doctor, but I would like to toss in my $0.02 anyway.

If after all this time you are still symptomatic to any extent, then
there are at least two other possibilities:  One, the bugs are resistant
to Z-Pak; and/or Two, the main cause of your sinusitis isn't a bacterial
infection at all.

Either way, I don't see a reason to keep on using the Z-Pak at this time.

The real issue is why you get up to 4 sinus infections per year.  The
fact that you're getting them long after your dentistry might indicate
you have a longstanding physical problem in your sinuses.

You not only need a CT scan, you also need to take that CT scan to a
qualified ENT who can interpret it properly.  The ENT may also do a
nasal endoscopy to view your sinuses directly.

I really find it irritating the way some people try to address discussions to
the doctors only.,,,,,,

The fact that this came on following the dental problem suggests that there is
a problem at that location
could be a pus pocket
could be a minute opeing
could be a whole sinus infection wtih thickened material.
The real problem is why 4x a year. suggests you don't really clear the problem
in  the first place.
Best to get a CT scan see why the recurrence.

Sterilizing the irrigator once a week is usually sufficient.

Thanks to Steven and Dr. Grossan for the helpful advice. I had the
sinus CT scan and picked up the films. The report said the most
striking finding is that of "substantial infiltrate in the right
maxillary antrum". Here are the findings:

1. Substantial inflammatory change, right maxillary antrum with a
fluid level and marked mucosal thickening.
2. Significant sinusitis involving ethmoids, primarily on the right.
More minor sinusitis involving the sphenoid and frontal sinuses, again
more prominent on the right.
3. No bone destruction.

My doctor recommended that I see an ENT if the antibiotics don't work.
Can anyone here recommend an ENT in the Austin area? My doctor
recommended Dr. Peter Scholl. Has anyone had any experience with him?

You could see which University has an ENT Dept. with some prominent specialists
in Sinusitis.  That is what I would suggest.

I grew up with the GUNK you are decribing coming out of my nose regularly!
I have fragile capillaries in my nose, thus the blood and mucous. At least
thats what my doctor told me growing up!
I  couldnt take the Z-Pack, it made my sphincter bleed!
Perhaps its making your nose bleed!?
It is a side effect!

Thanks Preesi, that's an interesting possibility. I rarely get
nosebleeds, though, so it seems somewhat unlikely to me. Also, this
stuff only started after I had my wisdom teeth surgery four years ago.
I checked the list of side effects for the Z-Pack, and I didn't see
bleeding on there. Where did you see that? I was thinking maybe the
Rhinocort was causing the bleeding, since that has happened to me
before, but how do you explain the yellow pus?

I finally saw an ENT, Dr. Peter Scholl in Austin, this week. He said
there was no evidence of sinusitis on my CT scan (he disagreed with
the radiologist). Despite a slightly deviated septum, he saw no real
abnormalities.

Though my symptoms aren't bad, I am still spitting out bloody yellow
pus sometimes in the mornings when I wake up, and my right maxillary
sinus is often hard to breathe through. This is basically how I've
been for the past few years. I have finished the Zithromax and I am
still using Rhinocort. My allergist put me on Bactroban/Ocean nasal
spray to try to clear the infection, which I've been using for the
past three days. I also purchased a Hydro-Pulse machine, which I've
been using since Wednesday. Nothing but water comes out when I
irrigate, and I haven't noticed any change in my symptoms thus far.

How can I have an infection that doesn't show up on the CT scan? Is it
really an infection? Is there anything else that would cause the
bloody yellow pus?

It is possible that the benzalkonium in the Ocean is impeding your progress.
Ask your doctor abou tputting the Bactroban directly into the nose. That way
there is longer contact with the nasal membranes. Yours may be a nasal membrane
infection rather than  a sinus infection, it which case the bactroban ointment
applied directly may be more effective. Use the hydro pulse to clean the nose
and help restore the cilia.

Since you have a disgreement among doctors,  I think you need another opinion.

I would suggest seeing a sinus expert at a University medical center.
