Do I tough it out

OK-- another damn sinus infection.

Interestingly, it started about 4 days after I came off a 4-week course of
cipro due to an unrelated problem. But the weather has been changing here
and that is often when the sinus problems occur.

So, could the antibiotics itself have triggered it (I suspect not, but I'm
curious).

My more basic question: I know my doc really doesn't like to prescribe
antibiotics. He said he does it less and less.
I'm trying to tough it out. I know 2 years ago I needed about 4-6 weeks of
biaxin XL.

At what point do I cry uncle??? (the sinus infection started about 6 days
ago).

try and keep the sinus passages open so they can drain and keep the
infection down.  Use Afrin, and do plenty of lavages.  If you are
inflamed, use a steroid as an antiinflamatory.  Ipratropium Bromide is
also good at keeping the mucus level down.  Also actifed or pseudophed
will keep the swelling down and keep you draining.

Steve and Fred:

I have found taking nasonex at the first sign of a sinus infection to be
very helpful at (I think) averting it.

But this time it got away from me. I am doing the nasonex, but so far no
joy. I might add my main symptoms are:

nasty sore throat; inflamed sinus feeling, general malaise.

I generally do not run a fever and I have no problems breathing (it doesn't
get stuffed up). In the past, I've tried the Grossan thing and also a netti
pot (which I prefer as it is easier to keep clean). Neither helped (I
realize many of you advocate using it) and sometimes it seemed to make
things worse. Can it spread the infection?

Steven-- you mentioned waiting 3 weeks. Were you just being flippant or is
there some basis for that response? I did figure on waiting 3 weeks but it
is much worse now.

 He's referring to a poorly done study in JAMA that concluded that
antibiotics were of no use in sinusitis. A closer reading of the
study, and the editorial, show that the study was underpowered and
treated many patients with viral illness. The editorial states that it
didn't clarify the subset of patients who would benefit from
antibiotics.
 Clearly, you are in that subset.
 The study was in the popular press, and widely quoted, but rarely
read or interpreted.

(Steve usually has more detail in his posts so I don't know how to interpret
it but maybe he could elaborate).

Anyway, update:

I did see my doctor today.  I was expecting to make an appointment for next
week but he is always booked yet had a sudden opening today. Anyway, he
prescribed Biaxin XL again. 2 weeks + a 2-week refill. I'm going to try and
hold out for another week. If no improvement, then I start the damn drugs

Most cases of acute sinusitis are self-limiting, but last up to 4 weeks.

I was referring to standard medical knowledge which is that most cases
of acute sinusitis are self-limiting but tedious, typically lasting up
to 4 weeks.

You're a patient.  So be patient.

And in general, medical standards are very, very lacking in quality.
