I guess the good news is he didn't just blow off my concerns.  He freely
stated that with my clean C-T scan, except for a small spot in the
frontal sinus, his unwillingness to do FESS with such a clean scan to
look around is purely a liability decision.

He did a mid meatal culture (I know, I know, but my ID doc at least
wanted to try) and will do sensitivity if anything grows out.  He didn't
argue about the controversy over C-T and symptoms, just has made a
decision that he won't operate where the C-T is clear.

I brought him the article on enzymes/phages to clear up biofilms from
Harvard with a note on it; "this worked GREAT on my hot tub lines, why
not my nose???"

He asked to keep the C-T article.

He was sympathetic to the adrenal suppression symptoms that even teensy
bits of infection cause me.  He asked if I'd be in his office talking
about surgery if I weren't facing transsphenoidal sinus surgery.  That
was a GREAT question.

The answer was "no."

Did you say you already had a complete immunological workup?  If your immune system is being suppressed by adrenal insufficiency, then it ought to show up as poor response when you're challenged with pneumovax and the like.

Since you have a history of chronic sinusitis, and your cultures have sometimes been positive, a good modern immunologist can order up special vaccines made of killed versions of those microbes to see how your immune system reacts when challenged.  My immunologist zapped me with both pneumovax and a specially prepared serum of killed H. influenzae, since I was a sinus sufferer and my blood showed antibodies for past H. influenzae infections I had earlier in my life.)

Many years ago.  Recently only allergy testing.

I'm a mixed bag, and in my case, my immune response is pretty robust.
Since quitting the vitamin D analog topical, I've stopped getting
numerous opportunistic infections and my sinuses are better, with mostly
less aggressive management than before.  I never catch flu or bad colds
or things my husband has.  If he gets sick, my sinuses get worse, I feel
like I'm fighting something, but I never get it full blown.
Even when I was more immunse compromised, I had robust natural killer
cell function (typical of Lyme disease) and some down regulated and some
up regulated Ig subsets.

I experience both highs and lows adrenally.  I feel AI with normal
cortisol numbers because my cells are accustomed to excess.

I've never had a culture. Yesterday was my first, and it was just mid
meatus.

Did that change your treatment or improve your sinusitis or health
generally?

Hi, Ellen,

I'm waiting for the expected negative culture results, and my sinus
infection has kicked up a bit the past two days.  I'm in an adrenal low,
just coming off of two bedridden days but on the upswing to half normal now.

My sinuses were feeling better than they had in years until I resumed
irrigation after a couple of days off, even though I cleaned the
irrigator first.  I'm thinking that the tubing of the Grossan harbors
pathogens no matter what, and I'm thinking of using my much more
cleansable NeilMed bottle from now on, instead.

I'm sorry for the low, and hope the high has some staying power ;-)

for some H2O2 use.....

After I do the irrigation solution, I run a full reservoir of water
through the machine (desalination) WITH a bit of the H2O2 AND a bit of
the grapefruit seed extract.

Thanks, Neil. Problem is that the highs are very damaging, in the same
way that, say, long term Prednisone or dexamethasone usage is. If I can
just manage to get my pee/spit/blood tests on the high cycles, I can get
my chance at a cure through pituitary surgery.

I use it, twice in the past week.  In fact, when my husband had a bad
cold that made my sinuses smell and feel bad, I used enough to make me
feel like the back of my head might blow off.  It worked incredibly
well.  Can't recommend it as a cheap thrill, though.  :-)

I have bleach solution in it now.  I'm going to get into the habit of
doing this right before I use it each day, I think.  I wish the tubing
were replaceable, though.  Fatal design flaw, IMO.
