Polyps again (and some questions)

Hi there,

in the past weeks I've made a bit of a progress, but unfortunately the
polyps (or polyp - I'm not sure if it is one or two) are still a major
issue. I was using Nasonex but came down with a horrible sore throat and
subsequent cold or flu for a week during which I stopped the use of
Nasonex and relied on Sinusbuster instead (which actually seems to help,
too).

After another visit to my doctor I've taken up Nasonex again but the
polyps seem to actually get worse (today is the first day with Nasonex
and I haven't used the Sinusbuster spray - probably I still have to wait
a few days but even during the first use of Nasonex three weeks ago my
polyps didn't improve and I had the feeling that it actually made things
worse).

Is it common for polyps to continue swelling througout the day?

I wake up with no symptoms and can breathe/smell just fine. At around
3pm I notice airflow from my right nostril beginning to get worse (my
symptoms for sinusitis / polyps are limited to the right side). The past
days I was usually more or less blocked on the right side at about 7pm.
Today (after I switched to Nasonex again) I had blockage already at
around 5pm :/.
Usually any spray will not reach into the nasal cavaties when I apply it
in the evening (except with Sinusbuster I would use more of it so that
when I lay down and tilted back my head, some would trickle inside).

I'm just wondering if this is normal or if there is some kind of
infection / inflammation going on that would require either antibiotics,
or immunotherapy for allergies (which I've been told I can't start
before autumn when pollen season is over - meanwhile I've discontinued
the use of cetirizine because it had absolutely no effect on my polyps
but resulted in heavy drying of the mucus). Or is there something in the
mucus coming from the sinuses that makes them swell?

Apart from that I'm not really satisfied with the ENT I'm seeing at the
moment. He schedules very frequent follow up visits (around every 10 to
20 days), performs ultrasound to check the sinuses and usually comes up
with a result I could've told him myself (it's been getting better a bit
constantly but my right frontal sinus still hasn't cleared completely -
which I can tell if I press on it). And despite the high number of
follow ups he has the habit of scheduling the next appointment so that
the steroid sprays are usually empty a couple of days before it.

Recently I've been wondering if conventional polypectomy makes any
sense? Or is it more or less useless due to the high recurrence rate?

Have you had a trial of prednisone with antibiotic?
Any infection will enlarge nasal polyps, as will salicylates. If the
cortisone spary isn't working we usually go to oral prednisone with
antibiotic.
