Hi
The skin on my cheeks, in between my eye brows and in the eye brows,
in the ears and the worst place is the scalp is always flaky. It does
vary. It has been likened to cradle cap. I've always had it.
Also I've always had sinus problems. Most of the time I don't bother
with antibiotics as 10 - 15 days of pain relief is enough until they
are less painful. If I have 10 - 15 days of antibiotics I would still
have to take pain relief and I would feel the same in the end. I don't
think the antibiotics were having much of an effect. Over the years
I've had 5 operations. The initial ones did help as I had a deviated
septum and swelling causing blockages. I had a lot, huge amounts, of
not so wonderful discharge after those the first ops, some if it the
colour and consistency of caramel but that was about 18 years ago. A
consultant told me that the sinuses show an allergic reaction but I'm
not allergic to anything.
When I was a child I'd have the most horrendous ear aches and I still
have very mild pain in the left ear, most of the sinus problems are
felt on the left side.
I think the only time have been able to fully breath through my nose
and been totally pain free is just after an operation and it seem to
only last days maybe a week, then I can feel the congestion coming
back.
I suppose one of my problems is that if I've always had it how do I
know what normal is :)!
That's the background.
Would anyone be able to shed any light on whether these conditions are
related? I have looked into Seborrhoeic dermatitis (cradle cap) and
this could be caused by fungi. Can Sinus problems be caused by fungi?
The left ear is a minor thing.
I would like to try and solve (ultimate wish but unlikely) or manage
the sinus problems without more surgery. I have had a very bad bout of
sinus problems. Doc prescribed Doxycycline 7 days, as soon as I
finished them it felt even worse. Then it was I was prescribed
Beconaze, Sudafed, loratadine and clarithromycin. Then different
antibiotic and now another one, in all about 6 weeks of antibiotics. I
have finished the antibiotics. I get a very bad headache when using
Beconaze or Synartis corticosteroids nasal sprays. When I had
clarithromycin I had a reaction to it on day 9 of 10, had black
tongue. I seem to have a reaction to half the drugs the doc has
prescribed.
I also use sterimar nasal spray and steam on a regular basis. I've
never irrigated. My sinuses don't seem to affected by the seasons.
Thanks

Rob,
Have you considered allergic fungal sinusitis? The most current
Mayo theory is that it is not an actual allergy to mold, but an
abnormal immune response (probably genetic) to the mold. They call it
eosinophilic fungal sinusitis. They were saying at one time that most
if not all chronic sinusitis is caused by this, but I think they are
backing away from that now.
But it sounds like your case very well could be that. I have recently
been diagnosed with this after years of antibitoics not working very
well by themselves. I also had a lot of the dandruff like problems and
itfchy skin and started getting vaginal yeast infections when I was 12
and had never taken an antibiotic then. From what I have read it is
an abnormal T-cell response to fungi in general although I don't think
it shows up on a standard immunology test.
There are different schools of thoughts on the treatment. The
current theory is that using an antifungal sinus rinse such as
amphotericn B or itraconazole (sporanox) helps significantly. But they
are also working on a study using oral sporanox (systemic). You might
want to do some more research on this and try to find a doctor who
treats it. There are others besides those at Mayo. For me the rinses
didn't help at all; just seemed to cause more irritation. I think the
systemic ones (I'm on Vfend now) are working much better. Sometimes
you have to take a combination of antibiotics and antifungals to combat
the problem. You don't want to get so bad you have to take oral
steroids. That's where I am now.
And yes, ear problems can be caused by sinusitis. During my
last surgery the doctor saw the fluid running into my eustachian tubes.
Where do you live?

I live in the US: East Tn., but had to go all over the country to get
diagnosis and treatment.; not much help around here.

Yes, they can.
But there's also a rare condition called Wegener's granulomatosis, whose
primary symptoms include both sinusitis and skin lesions. You may want
to be tested for that condition.

Another symptom of W.G. is chronic ear infections.

As another poster has stated, this sounds like seborrheic dermatitis. The
primary cause of this condition, frequently associated with dandruff, is a
hypersensitivity reaction (allergy) to a fungus called Pitysporum. The
best treatment for this is 2-fold: an antifungal ketoconazole shampoo
(Nizoral), a ketoconazole (or other antifungal) cream for the eyebrows and
affected skin, and a mid potency steroid cream to reduce the flaking and
inflammation while the antifungals are doing their job. All of these are
prescription items, and are best obtained by seeing a dermatologist.
Since a lot of sinus inflammation seems to have an allergic component, I
suppose it's possible that these conditions are related. In my case, I get
sinus headaches from a deviated septum and pooling of thick mucus when I
sleep. I can pretty much anticipate a sinus headache later in the day when
I wake up with a stuffy nose and hives (urticaria) on my forehead, temple,
or eyebrow on the affected side.

I had terrible ear infections as a child, I can still remember two occasions
when my ear drum ruptured. That hurts! I also have the itchy scalp, it
drive me crazy.
I was diagnosed with eosinophilic nonallergic rhinituis about 3 years ago.
My doctor has the same thing so I know that he stays on top of the research.
He diagnosed me in about 2 1/2 hours on my first appointment. He did the
standard skin tests and told me that I wasn't allergic to anything, I knew
that. Then he took a Q tip and stuck it up my nose and made a slide. It
didn't take long for him to tell me what I had from the slide.
I have had two surgeries. Right now I am feeling a bit better, I live in
Iowa. I don't know if the weather has anything to do with it. I have found
two things that seem to help me. A while back there was someone discussing
using Pulmicort Respules with saline to do a sinus rinse for 5 minutes on
each side. That seems to open things up and helped me quite a bit. After
using NeilMed saline rinse and Pulmicort and itraconazole for a while my
ears started to bother
me. So I decide to stop doing the NeilMed since it didn't seem to do much.
I noticed that when I used the Pulmicort for 5 minutes after everything else
I would get lots of stuff out. So I decide to try using the itraconazole
for 5 minutes and that seemed to work much better. I think that leaving the
itraconazole in longer has made a big improvement and I haven't used the
Pulmicort or NeilMed in several weeks. I don't always feel good at the end
of the day (12 hours later) but using the itraconazole seems to clean things
out well enough that I feel OK again. BTW I am living on Allegra D and
Nasonex and some other stuff but I do feel like I might be able to servive
with this.
I wish there was a little pill I could take and be well again, but no luck
so far.
All the best.

Thanks you so much to all that have replyed.
Sorry that I havn't replyed before. It's been hard work getting
through the last few weeks.
I've been trying different things from the doctors for seborrheic
dermatitis. Had a steroid liquid for the scalp which worked well
initially but end up hell as the seborrheic dermatitis returned about
10 times worse, wanted to rip my scalp off. Using an antifungal
ketoconazole shampoo. The doctors prescribed emollient cream for the
face, which I seem to be allergic to, had someone today ask if I had
sunburn, going to see if I can get some antifungal cream from the
chemist, we have hydrocortizone cream in the house, going to see if
that helps as well. But we'll try it in stages and see the doctor as
well.
See the ENT specialist next week.
------------------------------------------------------------------------------------------------
Update I've seen the ENT consultant.
It put me in a much better mood.
Sorry if the details are a bit sketchy. I get very nevous when seeing
medical professionals also I he was examining me at the sametime as
talking. My partner, she was there taking notes but I haven't spoken
to her since.
He confimed the reason why my face looked like I had sunburn was
emollient cream. He said something about using that type of cream was
like lightly sandpapering your skin, and for me it felt like it!
Also confirmed that what was happening to my skin on the face, scalp
and ears was also happening to the inside of my nose and sinusis. I've
forgotton the technical term he used, it has been mentioned here as
well. Basically I show a allergic reaction but I'm not allergic to
anything, as far as we know. Have had the skin prick test done a few
times before.
My sinuses look good but I have to have a CT scan done to make sure of
the ones he cannot see.
The best bit is that surgery is unlikly - fantastic news, I was
dreading more surgury.
After the CT scan, providing it is clear, as far as I can remember,
there is two avenues of investigation. 1. Allergy specialist 2.
physio.
The physio I found very interesting - something to do with possible
tension or something not being right with the muscles. He press the
muscles at the back of my neck and the side I have sinus pain hurt
very much more than the other side. The odd thing is that I feel okay
when in bed and sleep very well, most of the time. He ask a lot of
questions about my teeth and jaw but they seem to be okay.
It will be interesting to see what happens?

Did you try the cortizone-10 for the itchiness on your scalp?
I have Celiac disease and it affects my skin like getting little boils
and so itchy that I scratch until they bleed. My scalp gets itchy first.

Don't automatically assume that if the CT scan comes back looking
"normal" that it doesn't mean that you still don't have sinusitis. If
you do and you let it go too long, you will end up in "big" trouble.
What did the ENT suggest for the "allergic" type reaction. What did he
say you are reacting to? If you don't remember, and he "blows you
off" you need to get a copy of your records and CT scan and find
another ENT ASAP!!
