A bone scan is just that--it images the bones of the skull. It wouldn't
show inflamed sinus linings, polyps, cysts, all the soft tissues that
can be involved in sinusitis. That's what CT scans are for.
You get a bone scan if extensive infection of the bones of the skull is
suspected.

An endoscope can't detect a bone infection because the endoscope can
only see the tissues and mucous membranes on top of the bone.
A fungal infection might be visible with the endoscope, but I believe an
MRI is called for if a fungal infection is suspected.

My ENT did find my bone infection: only because dead bone was
protruding--and it took him a while to find it. Endoscopy is very
operator dependent.
Bone scans just visualize bone activity: so if a bone is infected or
inflammed it "lights up". It's more sensitive than a plain film for
finding bone infections or cancers. They are helpful in suspected
osteomyelitis.
I've heard that in fungal sinusitis the sinus contents are thick and
dark--the consistency of peanut butter--so you may see it on an
endoscopy. The more subtle inflammation in response to fungal
presence--the Mayo Clinic idea--wouldn't necessarilty show up.
A sinus CT is the preferred method of imaging the sinuses at this
point. MRI's aren't as good.
