Hi, just wondering if anyone here could sugest what might be causing my
problems. Ive been the doctor and he has done blood tests and found nothing,
well he found high cholesterol and seem to be obsessed with that to the
exclusion of everything else atm..

I constantly feel like there is somthing running down the back of my throat
and i need to swallow it. Sometimes it is worse than others, but it's there
almost all the time. Right now its been really bad for about 2 weeks.

When i wake up I almost always have some kind of ichyness in my ears.

I've been feeling very lethargic, having trouble concentrating. And often
feel fuzzy headed.

I get heartburn quite alot now, I never used to get it. I used to have a
cast iron stomach, now i get up have some cereal for breakfast and off it
goes. It doesnt seem to be related to any specific food, and it comes and
goes, but i just find it odd that i never used to get it and all of a sudden
a coupld of years ago i started.

This has been going on for almost 2 years, the doctor doesnt seem interested
as the blood tests didnt show anything up. He has spotted mild ear infection
a couple of times but told me it was nothing to worry about and that it
would go away on its own.

Oh i'm 31, male, and a couple of stone overweight.

I heard that dairy products can cause excess mucas and excluded them from my
diet for a week but it made no obvious differance. Tbh i eat very little
dairy anyway.

Any ideas on what could be causing this and what i can do to investigate
further?

Sounds like two things might be happening; one is possible allergies,
causing itchy ears and post nasal drip.  Can also cause grogginess,
fatigue, mental fog.

The other is the stomach thing; lots of folks (pretty much everyone I
know of) with GERD finds it disappears on a restricted carbohydrate
diet. You could try just dropping or limiting starches and sugar to see
if that alleviates it.  It could be triggering your other symptoms, too.

I had some of your problems, including allergies.
Post nasal drip that never goes away. I have to use saline rinses
(sometimes 2x a day) otherwise I get infections. Upper respiratory
infection and if I do not take care of that I've had pneumonia and
bronchitis too.

If it sits in your throat- gargle with warm water with salt in it-  in
the morning and night.
This rinses the flem at the back of your throat. Sometimes I have to do
this for a week.

Checks that should be performed:
1. Check for a hiatel hernia.  A barium swallow or endoscope will
reveal this.  An xray may show it up.
2. Check for acid reflux.  Pepcid AC can help here twice a day.  A
barium swallow will usually show some reflux.
3. Allergy testing.  Obviously you could have some allergy, maybe mold
as the house is closed up in the winter.  You can pay to have the
indoor mold count done in your house.
4. Excessively dry house, do you have a humidifier running and an
indoor humidity greater than 25% RH, preferably between 35% and 40%.

What is your blood pressure in the morning when you first wake up, or
during the night if you wake up?  What is the BP later in the day when
sitting and resting 30 minutes?  Do you snore a lot at night and is it
loud?  Do you drink alcohol?  When is your last food or liquid intake
in relation to bed time?

My layman's recommendation for the short term until you can see the
doctor again is:

Mild night time acid reflux can be helped with pepcid AC and raising
the entire head of the bed 3 to 6 inches.  Simply cut a couple of 4x4s
and put a slight hole in them for the feet of the bed frame at the head
of the bed.  The entire matress must be on an incline.  Simply trying
to elevate the head with pillows can make it worse as it pushes the
belly upwards.

The snoring and tiredness along with elevated BP that is higher in the
morning can be a sign of sleep disturbances from snoring, hypopnea
(abnormally slowed breathing), or apnea (complete pauses in breathing
lasting more than 10 seconds several times an hour).  The snoring can
also aggrevate the sinuses due to the vibration and cause increase
drainage at night.  Recommend you find a way to video tape yourself at
night witha  good microphone at the head of the bed and a good low
light camera.  Then look for how often you are snoring and any signs of
missed breathing.  Document it and take it to the doctor so you can get
a referral.

Do not eat or drink anything but small amounts of water within 2 hours
of laying down for bed time.  Try and do aerobic exercise 2 or 3 times
a week for a minimum of 20 minutes.  You want to get your heart rate up
to around 130bpm if your resting is around 70 and keep it near that
level for 10-15 minutes.

Keep checking your blood pressure several times a day.  If you need to
buy one of the good automatic BP machines, then do it.  (not the $49
Walmart special)  You also should take it in and have the calibration
comapred against a good nurse manual BP reading.
