My ENT prefers the quinolone class of antibiotics for sinus infections.
Probably the cheapest one in that class for sinusitis is Cipro
(ciprofloxacin).  My ENT prescribes Levaquin (levofloxacin) for me lately.
Other ENTs swear by Avelox (moxifloxacin).

Just be aware that the quinolone class has some annoying drug and
vitamin interactions, and some side effects you may not like.  But those
are the "big guns" when the first-line antibiotics fail.

I wonder how long one waits to use the "big guns"for an acute sinus
infection?"I have
had this sinus infection for over a year. (aside from chronic)  In the
past  most
of the cheaper antibiotics have quit working for me. I am afraid of the
stronger antibiotics
fearing they will damage my immune system even more.  I am using the
nasal irrigator
but I am convinced that when I use the water pik system, even at a low
setting it
pushes the pus deep in my sinuses. I have not used antibiotics for
about 4 years
now but may have to for fearing the infection will go to my brain or
eye.

I think you've waited long enough.
