I am looking for a good ENT or ID doctor that knows sinuses. I was going to
the Univ. of Penn. and my doctor says he can do no more surgically for me
so he can't help me anymore. I keep getting sinus infections everytime I
go off the antibotics for more then a week I had stenotrophomonas and
Pseudomonas aeruginosa cultured from my sinuses in the past 3 months. I
need someone to help me fight these infections so they go away or at least
more managable. I live in central New Jersey about 1.5 hours from both New
York and Philly. If anyone can help it would be great I am lost.

Couple of points:
My ENT has told me that recurrent sinus infections like you're
describing sometimes mean the infection has deeply invaded the bone
(osteomyelitis). In that case, long-term IV antibiotics seems to be the
best treatment--but even that isn't a guaranteed cure.
Are you doing nasal irrigation? We've had posters on this NG complain
that Stenotrophomonas is in their tap water, and that Pseudomonas
aeruginosa may be growing in their nasal irrigation equipment. Try
replacing all your irrigation equipment. I mean replace it, not clean
it. And use only distilled water for irrigation from now on.
I don't know any better ENT department than U. Penn in your area. Would
you be willing to travel to another part of the country?

Yes U. Penn is one of the best sinus surgery centers there is, but not
necessarily the best in non-surgicil areas.
I woudl agree that the original poster should try a different appraoch by
going to Mayo Clinic in Rochester Minnesota (or another excellent place)
Mayo is great in areas including internal medicine and infectious diseases,
which could be involved here. They accept all insurance (I believe they say
every single insurance company in the country) and are in a pleasant small town
where guest houses or hotels are not expensive...

I use a neti pot and boil my water and soak the neti pot in hcl acid every
few days. When I first got Pseudomonas aeruginosa a few years ago I
started doing this and gave up my water pik. U Penn. is good for surgery
but now it does not seem they want to do any more for me. I asked for a
referral for an ID doctor there and he gave me a name of a ID doctor that
he said knew sinuses but when I called the doctor was not accepting new
patients. So I called again and informed them that their suggestion is no
good and asked for another, I am still waiting for a reply.

Like I said, you may need to consider traveling to another part of the
country. Don Brady is correct that Mayo has great interdisciplinary
medicine, but you may want to wait till the spring to visit Minnesota. :-)

I tried to send you an email, but it came back 3 times. I'm going
to repeat what I told you before. You need to find out what is causing
these weird infections. You need to try if at all possible to keep the
appointment with the pulmonologist. You can have problems with your
lungs and not know it until it has progressed too far to get
appropriate treatment.

I live in the Philly area and have seen 5 different ENTs within the last
3-4 years. I know that there is one that i would NEVER recommend
(unfortunatly he was also the first and so he did surgery) but i dont know
if i should list his name on the forum. i think though, that all the forum
users are educated enough that they would never use this guy. The most
recent ENT that i am seeing is the first one to take a culture from my
sinuses (nothing grew). I dont know if there are certain rules around
listing names and also, as every situation is unique, what was good or bad
for me, might not be the case for someone else.
what i have found is that these doctors seem to be good at some particular
focus and often have trouble "hearing" what the patient is saying. one
doctor worked at clearing up my deviated septum and opening up my sinuses
-- however this had never been my complaint. another asked me if there
was any history of sinus problems within my family. i said "no". within
2 minutes he said to me, "given the extent of the damage and your genetic
pre-disposion..."
i'll send you a listing of the docs that i have seen off list.
