Gentamicin Dosage

I recieved my Gentamicin and started irrigating. I just wanted to make
sure I was doing it at the right dose.  The medicine came with
instuctions, but not for use with the Grossan irrigator.  The instructions
say to irrigate with 20ml through each nostril using the syringe.

I read in an earlier post that I should use 40ml mixed into 200ml of
water.  

My bottle reads: Gentamicin 40mg/ NaCl 0.9% 500ml in 50.  

Does it mean there is some salt in it already?  If so, should I add a
little salt since I am diluting it in more water?  What do you usually
recommend your patients do?

Also, If I am diluting it for the pulsile irrigation machine, will it
still be potent enough? Perhaps I should try it both ways, with the
irrigator for two weeks and then the syringe?

Note: The company is Home Care Solutions, Inc. in Tampa FL.  They are
affiliated (apparently suppliers for)SinusSpray, which has a website.
Steven L. was inquiring where to get this medicine.

I can't understand the dosage you quote here. Most doctors add 40 mg total
to 200 cc for the pulsatile irrigation. You could ask the pharmacy how you
can get 40 mg of gentamycin out of the container. If the entire 500 cc
contains a total of 40 mg of Gentamycin, that may not be practical for
pulsatile irrigation use.

I checked and you are right, it is 40 for the whole 500ml container.
Moreover, I was instructed to only use 40ml of it at a time (20 in each
nostril), so that would mean I would only be getting about 3.7 ml of
gentamiacin!

I called the supplier pharmacy, they were very helpful. They said my
doctor ordered the minimum potency.  Their maximum potency is 80ml per
500ml.  They say that their studies have verified this as safe.  This is
closer to your recommendation.  But not really, because they are still
saying to only use 40ml with a syringe to irrigate, which would mean about
7ml gent per irrigation (still not enough).  

I told them that not only would I need a higher potency (their max), but
also a higher dose - not just 40ml but 200 ml per irrigation (a bottle a
day, not a bottle a week) in order to get close to your recommended
dosage, and also for it to be practical to use the irrigator.  They said
they don't mind filling the order but they have to tell the doctor that
that amount has not been tested for safety.  Oy vey!, he probably will
balk at my request.

So you see, not only is this stuff hard to get, it's hard to get the right
amount!  My ENT is in another city.  If he refuses to increase the potency
and quantity, can my regular doctor prescribe it?  He's usually flexible.
Does one have to be an ENT to prescribe this?

I think they are speaking of preparing the solution in advance. It is easier
and much cheaper to obtain the 40 mg/ cc vial and add the medication to the
Hydro Pulse .
If your doctor has questions he can call me.

Wow, that would be tremendous.  Could you e-mail me that number?

Is there a particular company that has medicine in vials?  Or is there one
that you particularly trust?

The two cc vials, 40 mg / cc are standard and are available anywhere. They
are off the shelf and don't need to be compounded.
