Hello.

I have a large polyp in the lower left sinus. My ENT says the sinus is
4/5's occluded. I also have significant membrane swelling in the right
lower sinus. Both lower sinuses are also blocked - with the drain hole
being about the size of a pin for each. The ENT wants to do surgery to
open the sinus drainage and remove the polyp. He says that the hole he
will make in each will be about the size of a nickel! Apparently, I
will not be able to go swimming without a nose plug due to the size of
the holes. Holes of this size would allow water to flood the sinuses.

Is this normal for the size of the hole after surgery?

Well it depnds on what procedure is used but that is very large.  It's probably
a  result of the particular procedure he is using.  Try to get the medical name
of it and post it here.  You can tell his office that you need a detailed
description of the procedures that will be carried out to file with your
insurance carrier or employer or whetever.

I have my suspecions that he is using an obsolete or unusual procedure..   I
would be getting some second opinions at a major University medical center.....

Yes, it sounds a  bit like a Caldwell-Luc operation.  No longer done.
