Dr. Grossman,

I have you seen you responding to many questions since i posted about
my question regarding "sinusitis and candida krusei" and its possible
role of hypogammaglobulinemia (cvid), lyme, etc.

over the post 5 weeks i have been having reoccuring bloody and green
discharge and am developing headaches as well as a feeling of cranial
neuritis.  i was coincidentally cultured for candida krusei at this
time by new pcp so i was wondering if that could be an active agent as
well.

can impacted sinuses, polyps, etc affect the cranial nerves?

i see my immunologist tommorow for a second visit and am seeing my pcp
next week before the holiday break.  i was thinking of asking him for a
ct of the paranasal sinuses and and then a refferal to an ENT that is
in his practice.

i would appreciate your expert opinion.  please read the post again.
if it is "over your head" don't be bashful in saying so.  i'm just
looking for some good advice not a hero (unless it's an italian hero).

what are your feelings on long term antifungal use?

Skull base osteitis following fungal sinusitis.

Aspergillus sp. sinusitis is not uncommon in immunocompromised patients
but is unusual in patients who are not immunocompromised. The disease
may occur as a saprophytic condition, as an allergic sinusitis or as a
potentially lethal invasive disease. The differentiation between
non-invasive and invasive Aspergillus sp. sinusitis is crucial and this
distinction is fully discussed. The treatment options are also
considered. Invasive disease requires aggressive treatment with
long-term antifungal agents in sufficient doses combined with wide
surgical excision. We present a patient who presented with invasive
Aspergillus fumigatus sinusitis and subsequently developed cranial
neuropathies and skull base osteitis. She was initially treated with
oral itraconazole (400 mg daily) for 18 months but due to lack of
response this was changed to a new experimental oral azole
(voriconazole) which was continued for a further 14 months. She has
since remained well for the last five years.

On the internet it is useful to respond to general questions. When it comes to
specific person questions, the potential for misunderstanding and
misinformation is extremely high.

True recent case: Woman asked one of those advice sources on the net about what
to do about a leg pain and told to put ice.
So she allowed a fracture to go untreated and the ice caused necrosis .
Even when I have a patient infront of me, with all the information possible, I
may not know what to advise. So, giving advice without the benefit of person to
person, etc can be harmful.

thank you for your honest and disclaiming reply.  giving advice WITH
the benefit of person to person, etc can be harmful as well.  (a joke)

Sure they are.  Each news server has it's own retention
settings.  There are some services that archive usenet
most notably google.

You're just stating the obvious and trying to make a disageement out of it for
soem reason. The poster would already know that there is a limited retention
period (oterhwise they would see thousands of posts)..

The poster was inferring that he/she had been censored or singled out for
deletion of his/her post.  That does not happen.

Most likely the poster's news reader does not redisplay posts that have been
read....

Incorrect.  Many, many people don't understand how usenet works.

Judging by the fact that the op did not know what happened to his/her
post, I don't think they inferring anything.  The post was missing and
they don't know what happened.

Yes people often do not understand that you cannot really give advice based
only on symptoms.

The CT scan and of course clinic examination is often far more important than
symptoms, which alone are usually highly ambiguous.

Nah you're not - don't worry about it - people are really just trying to help
but sometimes it may come across as being too negative.   Really you just need
to read in an implied grin.

Maybe google did drop it if it dors not show up in Google now.      If so it
was just a technical glitch.   Feel free to repost it if you want.....

well the original post is back up now at the bottom.... strange.  guess
a tech glich...

looks like the immunologist has declared me immunocompromised under the
heading of hypogammagloubulinemia accompanied by chronic sinusitis (and
acute pharyngitis, i'm pretty ill right now).  so i will probably be
starting IVIG as soon as all the t's get crossed, etc.

still have to figure out a way to drain my sinuses without surgery.  a
2nd surgery is a LAST resort.  have to get a new ct of sinuses and talk
to ENT.

i've been irrigating but they are too far back there (the mucuous) just
getting blood and that's it (besides sinus pressure).  probably got a
ton of polyps hanging around in there too???

Right the CT scan will tell a lot.

Once your immune system comes back up it may help clear some of the existing
siusitis over time.....

could be that whatever software you are using to view the posts has a
feature where you can 'mark articles as read' and then another setting
that says something like 'show only unread articles'. So, as soon as
you view an article once - you aren't gonna see it in your tree again.
there are many, many such settings in full featured newsreader
software.
