Some advice and Nasonex questions

Hi all,

I just found this newsgroup when doing a Google search for Nasonex and I
thought I'd make a posting here.

I recently relocated to San Francisco from Austin, and have been absolutely
miserable (for lack of a better word) ever since. My sinuses have been
constantly dripping and clogging ever since I got here.

One morning I got up and I had a horrible rasping cough and severe wheezing.
I made an appointment with a GP and he told me that I'd got a viral
infection, so he couldn't give me antibiotics. He prescribed an inhaler and
Nasonex, which he told me to start using after 2-3 days of regular Afrin use
(he told me that my nasal passages should be clear for Nasonex to work). The
Afrin and inhaler made me feel much better and I eventually stopped all my
meds. Then last week, my sinuses acted up again.

My nostrils would block one at a time and alternate every hour or so. I
eventually got fed up and started the Afrin again. My nostrils are clear,
but I have perpetual nasal drip. Even with clear nostrils, I can do away
half a box of Kleenex in a single day.

The doctor told me that I can use Nasonex for years and it might actually
help me reverse my sinus problem. I noticed that my nose would occasionally
bleed with the Nasonex, which is why I stopped using it. But I think I might
restart because my sinuses are beginning to affect my work and my life.

How exactly is Nasonex different from Afrin? And does it have any long term
side-effects? Can anyone suggest any other remedies?

I have found that nasal formulations are much more effective than oral ones.
Benadryl would make the problem go away during the nights, but the next
morning the sinuses would be back with a vengeance.

That is correct.

Aim *carefully" outward away from the septum and/or cut the dosageto one spray
in each nostril and you should be able to stop  the bleeding.   Do not squirt
multiple times - just once, even if you think not much came out.  It is not
really very dosage dependent.

I used to get a little bleeding but not any more now that I use it lightly, and
I find it is just as effective at "half" dosage.

Afrin is a decongestant that addictive if used  for more than 3 days so do
*not* use  it regularly. Personally I would not use it at all and do not.

Nasonex is a nasal steroid that is absorbed locally and is safe.

Normally none.

It is your best and safest bet (if you can avoid the bleeding).

if you are getting bleeding with a cortisone spray it usually means your
membranes are becoming too thin. I have my patients stop the cortisone sprays
when that happens.

I have found that it can bleed a little if you start getting the liquid (from
the spray)  pooling anywhere on the inside of the nose when you spray, so tat I
have to wipe it away.

As long as I avoid that (by spraying just once) it does not bleed ever at all.

a day) my sinuses clog up and then I can't even use the Nasonex.

I've only recently discovered that Afrin is addictive and I am beginning to
understand why. What should I do?

That (rebound congestion) is the symtom you get when you are physically
addicted to the Afrin.   There are various ways of getting off it.

You will have to get off the Afrin.

You could try getting off "one side at a time".

Or you can use prescribed oral steroids briefly.

Or you could see if you could put up with the blocked nose for a while while
coming off it....

This is called Rhinitis Medicamentosa - the medicine is causing it.
Here is a method of getting off of Afrin from my book,
How to be free of sinus disease- Permanently:

  A gentler solution is to gradually dilute the amount of drops you are using.
Take the nose drop solution and add an equal amount of saline or Breath.ease
XL.  Label this Bottle A. Use this for a week.  Now add an equal amount of
saline or Breathe-ease solution to an equal amount from bottle A. Label this
Bottle B. Use this spray for a week.  Remove one cc from Bottle B and add it to
bottle C with an equal amount of Breathe-ease.  Keep on diluting  each week
till you stop the drops. Since you are no longer getting the rebound effect,
this is a preferred method, and you avoid “More Drugs”.  The extremely
dilute nose spray with Breathe.ease XL can now be switched to regular
Breathe-ease XL without the Afrin or other product.  The advantage of
continuing with Breathe ease XL is that it acts to moisten the nose and
stimulate cilia action.  In some cases where the medication has been used a
long time, the cilia fail to resume good movement right away and the nose may
still feel as though it is stuffy.  Hot tea, chicken soup, and compresses to
the sinus area help.

Another unusual problem I have is that only one nostril get blocked at a
time. And these usually alternate during the course of the day, switching
every hour or so. Is this common at all?

Correct that is actually normal (but ideally it would not completely block on
the congested side).

My MD told me yesterday not to take my daily aspirin while I was useing
Nasacort.

Why?

In a few  people aspirin is implicated in rhinitis and polyps (samter's triad)
but that has nothing to do with Nasal Steroids (except that they are used to
*treat" rhinitis).

I have allergies/sinus, and have the same symptoms that you have. My
eyes water and get itchy too.

MD recommended Claritin for me and my hubby. It melts in your mouth.
Within 1/2 hour the nose stops dripping and the sneezing stops.

The other preventive like Sudafed, etc. just gives me a dry mouth.

You have allergies that give excess watering, sneezing, itchiness etc.

For those, antihistamines such as Claritin are a good choice.

But for those with thick mucus,  and dryness, anthistamins are often a net
negative, as they will dry the nose and sinuses out further.  For those people,
nasal steroid sprays are better.

Hi Shirley,

My experience with Claritin is that I have to take it regularly for at least
a week and build it up in my system for it to have any effect. Sudafed has
no effect whatsoever and Benadryl offers only temporary relief, but is very
inconvenient because of its sedative properties.

I think I am becoming addicted to Afrin. Today's the first time in two days
that I didn't bring it to work with me and I'm feeling another sinus wave
coming on.
