Do I have Prostatitis?

I wanted to share my symptoms and see if anyone here have/had similiar symptoms and if they were able to find a cure. It is a long post but I want to be sure to get as much information out as possible. I am 30 years old and mostly in good health. I quit smoking almost 4 years ago. I drink a glass of alchohol maybe only once a week. I don't drink any coffee and I don't eat sweets (chocolate, etc.) I am 5'8" and weight 145 lbs, so I am not over weight. I had a dislocated shoulder surgery about 2 years ago and had a pre-surgery exam and I was NOT diagnose with diabetes. So for the most part I am pretty sure I am not Diabetic. I got a STD/HIV test two months ago and was negative.
I read a lot about prostatitis and know that a lot of people have different symptoms and problems. I know most people have pain related problems but I don't have any pain. I don't have any pain sitting down nor anything like that. I don't have any feeling of sitting on a golf ball nor anything like that. What I do have is the constant irritation of a full bladder and having to hold my pee so I don't have to go too much.
My Problems: Most of my problems are urinary. I have frequent urination, mostly at night when i'm lying down. Sometimes I have to go 4-5 times an hour. A lot of the time just a few dribbles and then I have to go again a few minutes later. When I do void a decent amount, I have slow start, my urine stream is really weak and I never seem to completely void all my urine, and sometimes I have a split/spiril stream. There is also constant dripping at the end. Most of the time I have to push hard just to get a couple of squirts out at the end. Usually after urinating my penis feels wet, especially around the inside tip of the penis. I also feel when I cough, I release a drop or so of urine. I have a hard time getting an erection and maintaining it. Usually because the feeling of having to pee is what causes me to lose my erection. Sometimes after ejaculation I have a warm/heated feeling at the tip of my penis and sometimes a sharp pinch, but nothing really unbearable.
My History: About 5 years ago I had an acute attack. I had the urgent need to urinate with burning sensation when peeing. I don't even know how I got it because I wasn't sexually active for about 4 months. I went to the ER and they took a urine sample and told me I had a "kidney infection." They prescribed me with 10 days of antibiotics (forgot which one.) This was the first time I ever used antibiotics. All symptoms diappeared and I was back to normal.
7-8 months later I had a similiar attack but not as severe. Since I had my first attack, I kind of knew that it was an infection. I went to the ER again and they took another urine sample and said I have a "Bladder Infection." They prescribed a week of antibiotics (I think it was Doxycycline.) Most of the symptoms diappeared except for the symptoms I have now.
2-3 months later I went to a urologist and he took a urine sample, measured my urine flow, and felt my prostate. He said I don't have any infection and said it is probably just in my head or stress, so I tried to adjust to it even though it has been annoying me for the past 3-4 years.
After searching the internet I heard about the term "prostatitis." Researching the condition I felt that it might be what I have; except for the pain. About 3 months ago, I found another urologist went to see him. He checked my urine sample, flow, etc. He gave me a my first prostate massage (very painful), and checked my prostate fluid under a microscope and said he found some white blood cells, so that means my prostate is inflamed. I prescribed 4 weeks of Cipro. He also said I should be a CT scan to make sure I don't have any physical problems that is causing my urinary problems. After the prostate massage, I felt great. I was able to urinate with a forceful flow and for once I felt I emptied my bladder, and I didn't have a full bladder feeling for almost 5 hours. After about two days the symptoms came back. I took the prescribed Cipro and it didn't help. I went to get the CT scan and went back to the urologist. He said for the most part, I don't have any physical abnormalties that might be causing my problems. He prescribed me some flowmax and gave me a second prostate massage. This time the prostate massage didn't help as much. The flowmax didn't help either.
Now I am just looking for another urologist/solution to try to cure my condition. Anyone have similiar symptoms and was cured please email me.

Finally found a MD in internal med who listened to my story and agreed to prescribe antibiotics when I felt I needed them. He agreed with me that with my long experience, I probably knew better when I needed them than he would with a lot of test.

Wonder if I need a different type of antibiotics. Have you ever had a culture done? It would best be done on expressed prostatic secretion (EPS), obtained by massage of the prostate. If this is done properly, you should be able to identify the bug and determine what antibiotic will work best. My bug has been identified many times as e. coli. It responds to a number of drugs including Cipro. But I have decided based on my experience that generic Bactrim works as well as anything else. I think Cipro is also going generic but I was paying about $5 a pill and Bactrim was a lot cheaper. Some will assert that antibiotics are only acting as an anti-inflamatory and that may be true in some cases. But in my case, I can actually see a white pus discharge from my penis when I am going through a bout. When I start having urinary symptoms, I will try to milk the urethra to see if I can see pus before I start antibiotics.

I wonder what to do but my thought is that food and water are really important. (when ABX can't do anything, I mean) What's more, I guess stress is not a good thing in our situation: I suspect my frequent urination being caused by bad thoughts ("is it a cancer ?, will i have to deal all my life with this ?...")
Have to simply wait and stand still !

I also heard of a treatment that is being tested in ucla medical center about botox injections in the prostate to shrink the prostate. I also heard some people had improvements with this treatment. Although I think the tucson clinic might suit me better because I really believe my condition comes from bacterial infection, since I don't have most of the pain related conditions lots of people seem to have. The botox treatment might be better for people with enlarged prostate. Well I really don't know, if one treatment doesn't work, I probably try the next. As soon as I can save up some money. I'll keep you posted! Have you tried any treatments? Anything seem to work for you?

People are spamming on the news group for a clinic? That's weird. Never heard of that. What I did was search on Dr. Polacheck's name and I emailed 5 people from this news group that stated they were going to go get treatment in Tuscon. One person said he was cured. Two stated their symptoms improved 60-80 percent but not completely. One person said he didn't end up going. And the last person said he only went for a one day consultation, but was able to relay the treatment information to his own urologist. He stated he was treated by his own urologist using prostate massages and antibiotics which greatly improved his condition.
I was initially thinking about going to Dr. Shoskes because he also used antibiotics and massages for some patients that might need it. But I emailed him and he was in cleveland which is too far for me. Tuscon is only 7 hours from Los Angeles, so I could possibly drive there. I checked Dr. Shoskes treatment results: http://www.dshoskes.com/results.html 16 patients were given the antibiotic and massage treatment and 9 were successful. Like I said before my problems all started from a bladder infection so I am pretty sure antibiotic and massages will help me. Have you been to Dr. Polcacheck?

They seem to think nothing is wrong, however they prescribe me Cipro anyways. If I have no infection why the antibiotics?

Why would I lie? I have no reason to lie. Like I said, I was thinking about going for treatment. I have never been to see Dr. Polacheck. Wouldn't it be a public service to post those horror stories for everyone to read. Why don't you give me the emails of those patients and I can contact them and find out for myself about their bad experiences?

That way I can find out for sure if I have some bacteria. Well my problems all started from a bladder infection so what else can it be? If anyone could recommend a good doctor who would do a complete prostate fluid test for bacteria here in Los Angeles, I would be happy to go see them.
Toby! Do you have prostatitis? Were you cured? If so please post the treatment you had and which doctor you went to. I am here to find possible cures for my condition. I'm not here to argue with anyone over bull.

Tommy - 27 Apr 2006 14:55 GMT Oh by the way. Here is an update on my condition. I been using an healthy prostate supplement with saw palmetto that I purchased from amazon. I also avoided all spicy foods and only drank alcohol once for the past month or so. So far my condition improved a little. I still have the constant urination, non empty bladder feeling , slow start, and weak stream but not as bad as before. The dribbling seem to improved some. I am going to watch more of what I eat and see if my condition improves more.

Watching what I eat and taking the prostate herbal pills everyday still didn't completely get rid of the symptoms of frequent urination and non-empty bladder feeling. However the dribbling seem to subsided. As for Tucson, I don't think I need to go to that clinic. I found another clinic here in Thousand Oaks, Ca. This clinic is about two hours from Los Angeles, so I might be going there since it is way closer for me. If anyone has experience with this doctor, please post here. Lets help each other out here.

Dr Zeitlin has an office in the Century City area (he used to have an office in Sherman Oaks, but it is no longer listed on his web site). Prostatitis is one of his specialities and he has worked with Dr. Shoskes. He'll take an EPS (Expressed Prostatic Secretion) sample and check it under the microscope for abnormal levels of white blood cells. I believe he can also arrange for 5 day cultures of the sample. I'm no doctor, but I believe that if there exists no abnormal level of white blood cells, infection is automatically ruled out (don't hold me to that).

I been to Dr. Zeitlin's office. He was the doctor that gave me the prostate massage as previously stated. He did take a EPS sample and checked it under a microscope. He did tell me that he found white blood cells. He wasn't able to do much for me except prescripted cipro and flomax, which didn't help. He never suggested a culture be done. I don't know why he didn't do a 5 day culture for me, but I'm not going to get another prostate massage unless the doctor will do the culture for me. Prostate massages don't feel good!

It sounds like to me that you will benefit from what I am about to tell you... First watch your diet.... NO SODAS..... Minimal amounts of alcohol.... Ok, the foods..... I eat all the red meat that I want.. mostly low fat red meat from the supermarket... I do eat hamburgers are places like (Perkins, etc.. the high grade burgers...).... not sh.t from McDonalds, Hardees, etc... Eat light amounts of Mayo only, cheese is ok preferably cheddar is what I do best with.... NO ketchup, mustard etc. they are full of vinegar and vinegar is a irritant.... (i do eat mustard now because it does not bother me due to me being at a 98% healed rate...) NO FRIES, baked potatoes are fine with butter and some sour cream.... Nothing with black pepper.... it sets me off.. not bad, just keeps my bladder from being relaxed..... NO soups... they are full of onion powder...... it irritates the f.ck out of me.... plus some other spices... Watch things like barbecue sauce.. they are full of spices and other irritants.... I do eat barbecue sauce now because I am able to and it bothers me only minimumly... You can add these later when your bladder is not irritated.... My urologist told me a little secret that most uros won't tell you.. Most will tell you to watch your spicy foods and soft drinks.... I told my last urologist to cut the bullshit... I want to know what the 2 main irritants are and he told me to mainly watch items with Tomato paste, citrus based items, peaches, pineapples, etc... He also told me to watch alcohol, but it has no effect on me at all. I can drink anything I want as far as alcohol goes with no bad effects.... he also told me to watch lemons... citrus again.. Watch out for imitation sugar or artificial sugars.... they set me off as well... I eat pizza and spaghetti and do fine with it as long as it isn't too spicy... NO bad effects for me now.. as I am nearly "cured" What I found out is that I needed to let me bladder supposedly heal.... you know... get it to the point that it wasn't inflamed... I also drink lots of spring water..... Ok, now for one of the most important parts of my little plan.... RELAX when you go to piss..... stand there and completely relax... Now you can try this if you want.. I know it sounds silly... but try it and see if it doesn't work....What I do is that whenever I am done urinating I will take both hands and rub the cheeks of my butt and it makes your entire body relax and watch what happens..... you will have more urine start to come out of your penism thus you are emptying your bladder fully... I know this sounds gay and all, but it does work.... also, a little trick that a urologist told me to do.... IN some men the little muscles in the urinary tract get week... thus some urine remains in the urethra... here is what you do..... Once you have totally relaxed by doing what I said above.. take your penis in your hand... hold it with your right or left hand hold it up.. take the other hand and feel just beneath your balls and you will feel the part right between both balls and give it a slight push and hold your flacid penis down toward the toilet and shake it.... watch the extra urine come out.... amazing.... :) keep doing this until it is all out.... What you are doing is clearing your urinary tract all the way.. The urine must come out. Now you will feel some wetness in your underwear.. this is ok... it is just the residual urine still coming out... Try this and see if I am correct...
