Hi.

I'm 32 years old and in faily good health.  Last week Tuesday, things
became a bit annoying - I had to go to the bathroom every 20 minutes
or so.  Dribbling also is a problem.

I went to Dr., who did a urine screen for things - nothing.  He was
perplexed, and finally asked, "have you been on a bicycle lately for
the first time in a while?"  My answer was yes, as I just started back
into my Spinning workouts.  He dismissed it as "annoying things" down
there and that it should go away.

It did for a couple of days, but it came back.  It's been back ever
since.  I've read a lot about this condition, and I think I may have
had it for a little while, though I've never had to go to the bathroom
as frequently as this.

It seems that, from time to time (once every two months or so), I
couldn't evacutate fully - requiring a "follow-up" trip to the
bathroom 10-15 minute afterwards.  One thing I can remember is - just
once - I had a super intense pain down there somewhere.  In my
reading, it was in my prostate.  I just didn't know it.  At the time I
was very stressed, and it hurt like hell.  I haven't had that again.

Is it possible I have a "chronic" case of this, and this recent
episode is a "flare up?"  Also, about two weeks ago (at the start of
this) I had an incredible need to go to the bathroom at night, but I
did not.  Call it lazy or whatever, but I just kept sleeping - I
actually had a dream about going.  That day things weren't good - it
was almost like I was "sore" or something.  Could this have pushed
things over the edge a bit?

I still don't have to get up at night to go, nor have I really ever
had that problem - only when I drink beer for a bit (as was the case
above).

Anyway, the Dr. is trying the 28 day Cipro regimen with aleve.  I'm
going to try Saw Palmetto for the symptoms.

Any advice?

Kpfeif,

    My symptoms started just like yours after getting into mountain
biking.  I did not make the connection however until my latest fare up
which has resulted in a minor but permanent condition.  Here is my
advice.  It is unlikely that you have an infection your problem is
more likely than not related to your pelvic floor muscles and nerves.
Do not take antibiotics unless an ejaculate, eps, and/or urine culture
indicates an infection.  I did not take these initial steps and
suffered through heroic doses of antibiotics with no significant
change in my condition, in fact, I blame failure to treat properly
with prolonging the condition.  If you get the above test performed
and they do not grow significant infectious organisms then do not take
antibiotics.  My recommendation is: 1. stop all bicycling; 2. avoid
all deep hip flexion activities, i.e., sit ups, squats, running, stair
climbers, etc.; 3. get your doc to give you high dose non steroidal
anti-inflammatory like vioxx, celebrex, ibuprofen; 4. avoid excess
sitting or if you must sit get a cushion that does not put pressure on
your perineum; 5. practice pelvic floor relaxation and find a pelvic
floor physical therapist.  It will take you at least three months to
feel better and you may never completely recover but you should
improve substantially.  I do not believe the flare up after cycling is
incidental and would bet your problem is related to your pelvic floor
as was mine.  Try to avoid all the trash advice on this forum related
to hidden stealth infections and auto immune conditions as the cause
of your problem.  Also, take herbal supplements with caution.  They
are expensive and in my experience do nothing of benefit.  Your
problem may be your prostate but I doubt it.  It is always best to be
conservative yet diligent.  I wish you well and post follow up if this
advice is helpful.
