Since I've been going through this for almost a year, it's starting to
really wear me down emotionally. I've got pelvic pain, very tight
muscles in the pelvis (according to the physical therapist I was
referred to), penile pain (down both sides of it starting just under
the head), and difficulty starting or maintaining an erection,
including no morning erections (I used to have them all the time). I
do have urination frequency, but no problems voiding and no stopping/
starting of stream. My PSA was 0.97. I've had MRI's and CT's of my
pelvis and abdomin that came out normal. None of the ED drugs (I tried
Cialis and Levitra) did anything for me. The erectile problem
definitely feels like it's related to the muscle problems (I feel a
pressure in the pubic area when I try to get and even when I have an
erection). Although I don't have any of the urinary problems outside
of going often, could this still be prostatitis? Doctors are quick to
say it is, but I think they just say that when they don't know.
Thanks again - I'm desparate here so any advice or comments would be
greatly appreciated.

Hello JD,
I know how you feel - that is having some skepticism regarding the
doctor's diagnosis. Even so, they are usually correct and I would
guess that this case is no exception.
As you may be aware, the term *prostatitis* is a blanket term that
covers several conditions that share many of the same symptoms. In
general the term is used to describe enlargement of the prostate. If I
recall correctly, prostatitis can be divided into two general
classes---those that involve bacteria, and those that do not. In many,
if not most cases there are no bacteria present and antibiotics are
useless. BPH is probably the more common version and stands for Benign
Prostatic Hyperplasia. I don't know the percentages but many men
suffer from some degree of BPH. In some of these cases, the disorder
causes mild symptoms such as frequency, split stream, urgency, etc.
However, in other cases an individual might experience many of the
symptoms you describe. Of the symptoms you mention, it is certain
prostatitis can cause--- penis pain, frequency, pelvic pain,
(especially between the anus and testicles), and erectile dysfunction.
The bacterial type might have all of these symptoms and fever. In
addition to the symptoms that you have there are potentially others.
Prostatitis can result in all kinds of different symptoms, and then,
each person experiences the know symptoms a bit differently.
You might also know that there are medications to shrink the prostate
(i.e., FloMax, Avodart, etc) and if bacteria were found in an EPS
(Expressed Prostatic Secretion) then antibiotics would clear it up
quickly. Prostatic massage has good results for some men and you can
actually learn to do it yourself - fun-fun.

Aloha JD!
A critical piece of information for us would be to let us know if
you've ever had any type of surgery to help loosen the muscles or
lower your pain, so if you'd just add a short post telling us what, if
any, that would be GREAT!
I'll take it that you haven't since you didn't mention any surgeries,
OK?
Now remember, we here have gone through hell and back a few times
(some more than others and some are in hell right now) and many are
dealing with many types of CP (chronic pain) and have had to learn to
"live as well as we can" either by having a good to great pain
(medication) management doctor or an understanding primary care
physician. In my case, I've been uncannily fortunate as I 'only' had
to suffer for the first (and probably the most critical) 18 months of
the last 10 1/2 years of my CP due to "failed back syndrome" due to
almost "third world surgery" here in Hawaii (3 of 'em!)... REMEMBER:
We are PP's (Professional Patients) only, no one, I believe...I could
be wrong about one person, has gone to medical school and has gotten a
degree in medicine. So, in essence, what you are going to get from us
is advice, shared experiences, support (galore!) and when it comes to
helping you diagnose something, all we can do is dip into our "Pool of
Vast Learning" and give you our best 'guess'.
If your doctor(s) are only telling you that you've prostitis and
they've run all the proper tests and have some sort of documented
PROOF that you have it, I'd have a fairly good feeling to believe
them. Now...if what you've said is true, and I take it that they've
not even given you a digital prostate exam, or if so and even THAT is
A-OK, then I'd NOT feel good about believing them because during the
first 3-4 years of my dealing with herniated discs at the L5-S1-L4
region of my back, I too ran into 3 knife-happy, surgery is the only
way type of Neurosurgeons, Orthopedic Surgeons and me always being a
healthy never-having-had-any-surgeries since I had my appendix out
when I was 2 years old ROOKIE and totally naive, greenhorn at the time
person...I let them take over my body! I completely trusted every ONE
of the 3 surgeons with their false promises and positive-only ways of
talking to me...I wised up REAL FAST after the 3rd surgery.
When I woke from THAT one, I was in so much pain I literally couldn't
stop screaming from the pain and could barely catch my breath! I
honestly thought I'd died and woke up in HELL for the first 5 to 10
minutes! That ordeal scared the crap out of me so bad I refused to
have any more surgeries unless my life depended on it. Well, as it
turned out, that surgery was also a failure and when that surgeon and
the other two caught 'wind' of my plight, they all convened at my Pain
(Meds) Mgmt. Doctors at my next appointment and told me there was
nothing more they could do for me as I was now pronounced with having
what back surgeons fall back on when they don't KNOW what else to do
(even if there IS something else they could do!) and told me I had
Failed Back Syndrome. Very convenient, yeah? They told me that the
ONLY thing to do now was to treat my pain levels properly with pain
meds (they then knew I had THE BEST doctor for pain management at the
time) and hopefully I'd be able to stay productive as long as I could.
They didn't mention the depression I'd have, or the muscle spasms, or
the ever-present sciatica pain in my left leg that just about drives
me MAD sometimes...or that I'd be taking a dozen different medications
within the next 5 years just so I could cope with the pain and deal
with all the aspects of how it was going to change my life and erase
all the future plans I and my wife had made together the past 21years
(at that time...We will be married for 29 years next May).
The last thing I want to see you go through is actually believing any
doctor/surgeon that thinks your problems can only be fixed with
surgery, surgery or surgery!
After reading what you had to say and getting the 'feeling' that you
don't really believe them and there diagnosis...this is what I'd do,
JD:
The next time you see whichever doctor you have the BEST relationship
with, request them to give you a testosterone level test! YES. The
very symptoms you are suffering from are the symptoms a man would have
from having too low a level of TESTOSTERONE!! Relief could be as
simple as getting an injection of "T" (testosterone) either once a
month or every two weeks! Believe me, because of all the trauma and
all the narcotics and non-narcotics but still very powerful
medications I've eaten over the course of a decade, my fairly new
regular doctor decided to check my "T" levels for the hell of it and
told me that my general feeling completely lousey all the time could
be because of having a low "T" level...I just about fainted when the
results came back and I was at a free "T" serum level of 86 (or 84)
when a man my age should have at least a "T" level of 350 to 1300!!!
Plus, I know I've always had a higher than average "T" level because I
had it taken as a routine "test" when I was 21 (or 22) and
hospitalized for a swollen lining around my heart. Well, THAT
explained away an entire HOST of symptoms! Libido, feeling shitty in
general, high pain levels and much much more...some things I learned
later that I had NO IDEA was affected by that wonderful hormone!!! So
now I get an injection every 2 weeks by a pretty nurse and believe me
when I tell you this: It almost instantly gives me a "feeling of well-
being", helps lower my pain level, even though I think about making
love constantly, it makes my drive so very much stronger that my wife
simply cannot keep up with me and I am forced to take "matters into my
own hands" (BWAHAHAHA!) and JD? I KNOW it will help you with your
erection problems because even though I was still able to achieve an
erection good enough to make love with, but NOW! Woooweee! The quality
has increased so much I forgot just how a "proper" erection should
feel, look and feel for my wife! (Yes, she's a happy camper now).
So, I guess I could have just told you to request a Testosterone Level
Test and leave it at that, however, I really did feel you needed to
know where WE are coming from and not to take what we say as you would
a trusted (key word: trusted) doctors. Plus, it always feels good for
me to VENT some! LOL... THANKS!
I hope you do this and that that is the problem in a nutshell (no pun
intended!). OK?

stop using them and only use honey,molasses, and sorghum.
The pain will slowly subside over a period of a few months.

Lay down, with your butt against a wall, with your legs straight,
up on the wall. Then let gravity help as you separate your feet,
cause your legs to form a "V" on the wall. Any problem doing
that? If so, which side is the reason?

Mr. j.d. you have already received some excellent advice
The only thing i would like to add is that you take something
like digestive advantage ibs everday, and maybe something like
threelac, and some high quality digestive enzymes.
always eat at least 2 apples everyday and all the grains you can eat.
If you don't have bowel problems yet, they are on their way
and this will head off the problem..
