article on Iridium 192 tip breaking at weld causing death ?

Hello all

New here, but not in the other NG.

Can anyone help me out here?

Several weeks ago, in alt.cancer.prostate.support ng, I saw an article
posted, where, during a HDR Brachytherapy session an Iridium 192  tip
broke off at the laser weld during withdrawal thru one of the
catheters and was not noticed. The article went on to say the patient
died within 4 days from excessive radiation.

Unfortunately, for me, I never saved it to a file :-((

Has anyone in this group read that article, and if so, either repost
it or provide a URL so I can review it?

I'm scheduled for HDR Brachytherapy about 6 mths down the track and
would like to see if my Clinic has this type of Accident included in
their QA Procedure. (Defective "O" rings disintegrate Space
Shuttles!!).
