Well, he suspects this may not be BPH but prostatitis.

He feels the sudden onset of much worsening symptoms and the type of
pain/discomfort I have is much more typical of prost.  So... he is
prescribing a 30-day course of Keflex (I cannot take Cipro or Bactrim, his
first 2 choices).

We shall see what happens.

DRE was perfectly normal and we did do a PSA 15 minutes after the DRE. I
said I hope the DRE doesn't artificially jack up the PSA but he said he
would only be concerned with a HUGE increase. Given my symptoms, in this
situation even a 1 point rise in the PSA would be explainable (if that
happened they would repeat the PSA after the antibiotics are done).

He does do PVP, but he certainly doesn't rush into things!! It sounds like a
cystoscopy and/or additional medicines (to shrink prostate) would certainly
occur before doing a PVP. (I'm not thrilled as I've finally decided that I
wouldn't mind surgery).

I might also add that he did an ultrasound and my residual urine was 10 ml
which is basically no retention (despite the sensation of retention).

About 5 years ago I had prostatitis, my PSA went from 4.2 to 7.0.  Didn't
know I had it when they drew the blood before my uro visit.  He was ready to
do a biopsy the day of my visit based on the PSA jump.  By the time I saw
him I had a low grade fever and slight pain in the rectal area.  He milked
the prostate onto a microscope slide and a few minutes later I had a
prescription for cipro. 30 days later the PSA value was back to 4.4.
