Reduction of Ejaculation after PVP

states "The only (patient) complaint is decreases in ejaculate volume.
That may be because we are sealing some of the ejaculation ducts during
the procedure.  We advise the patients about this."

I am scheduled for PVP Feb 23.  My uro warned me about this.  How many
of you who have had PVP's experienced decreases in ejaculate volume?
Did it affect your sexual functioning?  Negatively?

Yes, I have experienced a 100% decrease in ejaculate volume.  It's call
'retrograde ejaculation.'  I guess all post-PVP patients experience this to
the degree the bladder sphincter has been altered or damaged.

My understanding is the more the Urologist needs to work on the bladder
neck, the more the likelihood of retrograde ejaculation.

Did it affect my sexual functioning?   Yes and No.  No, as far as
functionality is concerned.  Yes, as far as the 'orgasmic feeling' is
concerned.  It's less.  It's different.  My take is that the heightened
feeling of sexual climax is a combo of orgasm and ejaculation combined.
Take away the ejaculation - and you're left with orgasm [and the resultant
sexual release].

My PVP was 3-months ago.  My return to sexual functionality began 3-weeks
after the PVP.  If pre-PVP climaxes were a '10' - 3 weeks later was a dismal
'2.'   It's getting better - maybe a '5' - even a '6.'

I'm getting used to the lack of an ejaculation.  My best description is an
orgasm followed by a slight dip or void feeling [where the ejaculation had
been].

However,  I made my choice.  I told the doc: "My priority is to pee good.
If I must choose between ejaculations and pee quality, I opt for pee
quality."  And that's what I got.

Am I complaining?  No.  I am getting used to it - and it's getting better.

I understand that retro may/will likely occurwhen there is work done on
the bladder neck.  My surgery is scheduled as "bladder neck sparing"
since that part of the uretha seems open but for some reason the TUMT
did not clear out the distal half of the prostatic urethra (the half
furtherist from the bladder).  I actually saw the opening on the monitor
when the uro did the cystscopy last week.  My understanding is that
there is a 20% chance of retro with my procedure but with bladder neck
sparing procedure, there is still reduction in ejaculation.  To those of
you who don't have retro, how much reduction was there in the ejaculate?
 How has that reduction affected your sexual functioning?

My experience is similar to James. But now almost 14 months post 2nd PVP, I
sometimes have a tiny or small amount of ejaculate. Still not sure of the
precise cause. Libido is also a bit less.

I too have retrograde ejaculation.  I find no diminishing of the orgasm but
there is some burning - particularly after a week or more of abstinence -
that feels like back pressure of the semen finding its way into my bladder.
I agree though that the tradeoff for better urinary function is worth it.

My experience with sensation is similar to James following a Holmium Laser
TURO about 2 yrs ago. The difference for me is that I believe the
ejaculatory ducts were sealed as described in this article as I see no
evidence of retrograde when peeing after sex. No big deal and the decrease
in sensation while regrettable was worth it to be able to pee properly!
Other parts of sexual function normal.

As I posted some time ago, I believe that my (2nd) PVP also sealed
ejaculatory ducts.    My questions are:
Does anyone know what happens to the semen or prostatic fluid when it's
blocked in this way? Where does it go  - if anywhere? And are there any
expected future problems from these fluids being unreleased?

the answer to the vasectomy question but i'm sure it's on the internet
somewhere. Semen comprises most of the ejaculate and with a vasectomy
it aint there anymore. The remainder is prostatic and some (Cowpers
gland ) which enters after the prostate ( I think ).
As far as I know there are no ill effects (like cancer ) from a
vasectomy.

Just for the record, vasectomy eliminates sperm from the ejaculate, not
semen, which is composed of stuff from the prostate, the seminal
vesicles, etc.  Sperm comprises less than 5% of what you ejaculate
before vasectomy.

I had the opposite effect after PVP.  For the first year it was lower volume
however after about a year the volume was much greater than before PVP.

I attribute this to less imflamation of the entire area after PVP.  Recall I
had seventeen years of imflamation/prostatitis/BPH.

My experience 19 months PVP is much like your other replies.  Right after
the operation I had retro and then I slowly experienced more ejaculate.  Now
I'm close to normal.  I have read that a tablespoon full is normal.  Prior
to PVP I produced much more then "normal".  The amount is no big deal.  To
me a small amount of ejaculate or a large amount does not feel all that
different.  A large amount of pee vs. a tiny amount of pee, that makes a big
difference!

Fifteen months after PVP I have gone from retro to back to almost normal
ejaculation, except that the ejaculate is like water or maybe urine very
thin and clear does anyone know what scenario this is.  "THANKS FOR ANSWERS"
