uh...oh... flomax no longer working-- advice please

Well, it looks like things are going south for me.

I've had BPH for many many years and I've been on flomax for about 4-5
years.

Lately, my stream is getting less and less despite now taking 2 per day.

I saw a PVP urologist, Dr. Wei in Ann Arbor, Michigan. That was in June and
things were going well then.

He had said at the time that flomax will eventually work less and then he
would prescribe a second medication to go with flomax. I have a message in
to him now (he won't get it until tomorrow).

My question: does anyone know what medication he might be talking about
(something, I think, that is used along with the flomax)?

Right now is a bad time for me to have the PVP done (I know it is never a
good time, but family and work-related things would really be a problem) so
I sure would like to delay things for about 3-4 months.

Of course, I'm sure I would have to go through tests (ultrasound,
cystascope, etc) before truly deciding if I truly have BPH and if I'm a PVP
candidate.

Oh, another question: when he examined me, based on a DRE, he said I had
about a 40 gm prostate. How accurate would that be? Does it probably mean I
don't have one of the huge ones that I've read about here that some of you
have?

Any advice would be appreciated.

Unless you have a manual job you could have PVP done in one day and be
sitting quietly back in the office the next day
PVP is a breeze and certainly better than the effects of the damm drugs and
i was only on them for 6 months too

I know. I somehow get the impression that perhaps there is some kind of
protocol that involves a second drug. But I'll find out when I hear from the
doctor, I guess. I left a message today and he will get it tomorrow.

It's just a bad time as we are planning a trip to California leaving on
2/23. I will be visiting my father who is almost 97 years old, so it means a
lot to be able to see him again (we are close). So, I would like to delay
things until after we get back (3/5). Of course, then I imagine there would
be all the nasty tests, etc.

But, yes, from my lurking in this group for many years I have said that when
the time comes I would definitely go the PVP route. I suspect the time is
close as my stream is GREATLY diminished!

(But, I have been in that situation in the past only to have things improve,
so we shall see)

Just a comment on the size of your prostate.  The overall size isn't as
important as how the prostate blocks the flow.  I had a relatively small
prostate, but it unfortunately was big in the wrong places.  I too had two
doses of Flomax a day for a year or so, but finally had to go with surgery.
No, it is never a good time.  But it is better to plan the surgery than to
have emergency surgery.

Philip,

When was that operation?  Are there distinguishable changes after that
(side effects, complete healing, etc.)?  I have heard there are ways of
surviving prostatic diseases (cancer as specified by Dr. Hennenfent's
book) but do you think the same processes would be effective to your
case, since prostatitis or other prostatic malady could be
predeterminant of prostate cancer?

Oh come on ...........everything from dry mouth to virtual impotence and
still getting up 4 times a night
PVP and within hours i could pee again without waiting for a dribble !!!

Now Nambucca--- that's not true for all.

I was one of the very lucky ones. I've been on it for years with NO side
effects.

Unfortunately, I fear the word "side" is now superfluous.

Dr. Wei's nurse called this morning. I had to repeat my message to her. They
are supposed to get back to me late today (which I guess won't happen now)
or tomorrow morning. So, I will see if there is some second drug. I'm hoping
to stall things a bit longer due to the aforementioned planned
vacation/visit (final visit possibly) to my Dad. It might be that the doctor
wants to see me.
That is a PAIN. He only sees patients on Monday (does procedures like
biopsies on Wednesdays and I think teaches and operates the other days) and
that is my busiest day. He is 120 miles away.

BUT, frankly, I would travel a lot further for a PVP as opposed to any other
procedure!

My uro has talked about putting me on proscar, so I'm interested in
what people have experienced. I'm on Flomax now, and there are hardly
any side effects. Mind you, I don't think it is helping the flow
either.

Been there...Your doctor has been reading the med journals that suggest that
Alpha Blocker and a 5-Alpha Reductase inhibitor (DHT) inhibitor in
combination are sometimes effective in helping to avoid surgery.   But in my
case it didn't help.   You can try a switch to Xatrol or Cardura, etc.,  if
you want to hold off on the surgery a bit longer.  Sometimes switching to a
different Alpha Blocker may have a short-term positive result in and of
itself.   In my case, by switching to Xatrol, I discovered that I could
titrate my dosage myself from the prescribed 10 mg to 15 per day...I did
that by biting the second tablet at approximately 1/2 way...it tastes
terrible, but I was able to have a whiz, so, was content though still pissed
off by all the "side"effects, (side effects= dizzy, lack of energy, 1/2 of
my internal strength, etc., but retro was not a problem whereas it was with
Flomax.) .   Finally, I got a PVP--because I saw it mentioned in this
newsgroup and did some intensive investigation on my own.  I am thrilled.
Had I known, I would have had a PVP sooner....and no, I don't own stock in a
PVP company of any sort.  Just an honest to goodness testimonial for PVP.
