Hi,
I just started taking Flomax and would appreciate opinions on the best time
of day to take it, morning or evening?
Thanks

I have been taking it a few days now and I am glad I take it after my
everning meal as it makes me drowsy.

Exactly. It has a variety of side effects which seem to vary
with the individual. It certainly made me drowsy and rather
lethargic.

I took it at bedtime.

Ditto, bedtime for Flomax and even more so for Hytrin.
Making it through the night is my objective.  If it wears
off the following afternoon, that's less of a problem
unless I'm out on the back nine--only porta-potty is on
15.  {{:-(

Look at it logically. It is a beta blocker so it will slow your heart
rate down to some extent.
Also, be careful if you ever will need to go off it.

I have been taking Flomax for quite awhile now and it has prevented me
from having to have surgery (laser or otherwise) for years. I would much
rather have a medical solution then a more invasive procedure.

For me there are no side-effects to Flomax whatsoever. This doesn't mean
there wouldn't be any for you but it might be worth a try before you
look into more invasive and potentially painful procedures that could
have irreversible side-effects.

PVP may be a medical breakthrough in many ways, but if it is not
necessary, why do it?

Griff,
I will give you one reason why to have a PVP sooner than later: to avoid
silent and possibly irreversible bladder damage.

Here are facts to consider.
Although your BPH symptoms are kept manageable by medications like Flomax,
the medication does nothing to stop the slow growth of the prostate nor does
nothing to actually remove the cause of your BPH symptoms.

With BPH your bladder is forced to strain. Over many years the bladder
muscles thicken.  Slowly over time the amount of urine left in the bladder
(residual) slowly grows.  The bladder slowly develops pockets and distorts
in shape as the prostate beneath it grows and pushs upwards against it.

Over time you will need ever increasing amount of medication to get the same
level of symptom relief.  Eventually the medications will fail to adequately
relieve your symptoms.  At that time, surgury is a necessity to remove the
blockage.

But if you have a PVP done sooner rather than later even before the bladder
walls have thicken, you will recover quicker, be less likely to have any
premanent damage and you will actually be removing the cause of your
symptoms.

If PVP was available 10 years ago and I knew then what I know now, I would
have had a PVP done at the time I started to take medications.

It is my belief that the availabiltiy of PVP has completely changed the way
BPH should be managed.  Instead of the traditional path: symptoms treated
first with medications, then increasing dosage of medications until
meidcation fail to provide relief,  then followed by a  TURP,  the new path
should be symptoms, short course of medications, then PVP.

I should share with the group a technique I have taught myself over the
years. I agree that straining in an attempt to empty the bladder is not
good at all. I have found that rather then strain, do the opposite:
relax all the muscles in that area and let gravity do its work. That may
sound ridiculous but when you learn how to do this properly it makes
total sense. It is easier to do when you are straight over the urinal
(sitting makes it more difficult) and mentally command all the muscles
to relax. Doing Yoga gives you a bit of insight in this area.

This doesn't answer the other problem you mention of continued growth of
the prostrate and I agree that sometime down the road a date with the
surgeon is in my future. Maybe by then PVP will be brought over the 49th
Parallel into Canada! One can only hope.

Griff,
I agree with you relaxation methods.  I practiced them myself for many years
prior to PVP.  It did delay the onset of going into retention.  But when the
time for surgury came, no exercises or medications were going to remove the
huge blockage I had.  It had to be removed.  Thank God (Laserscope and this
newsgroup) that PVP had just become available.

Good luck to you and best wishes for improvement,
Patrick

Re "Nambucca"

Pete is right. The only problem with Flomax is retrograde ejaculation in
some people.

In my case I had the additional side effect that Flomax sapped all my
energy.

It did not actually sap my energy all over but when walking (during the five
days that I took it before my heart rate went through the roof) it felt like
I was wearing lead shoes.

My problem with FLOMAX was rapid pulse. Had to discontinue use.
