Dr. Reiter at UCLA is my uro. He did my PVP for BPH in November
2005. He had me taking Avodart, not proscar, for about 3 months to
shrink my prostate prior to surgery. My prostate was about 110 grams,
but he said he would not agree to do the PVP unless my prostate was
below 80 grams. He was concerned about controlling bleeding I think.
When my prostate shrank to about 72 grams he performed the surgery.
I think he thought my bladder muscles were compromised because his
goal was to get my pvr down to under 200 ml (before surgery my pvr was
so high - around 700 ml - that I was self cathing about every 3-4
hours to try to protect my bladder)! We were both pleasantly
surprised when my pvr was below 50 ml within 3 months after surgery.
Dr. Reiter still has me on Avodart in an effort to keep my prostate
from growing. I have my annual follow-up with him in November and I
will see about cutting back and/or weaning off this drug. I think if
the result is that eventually I have to have another PVP it would be a
worthwhile trade-off. I really don't like the side effects of
Avodart. However, I am really glad I weas able to use it to shrink my
prostate so I could have Dr. Reiter do the PVP.

Very interesting and thanks for the report!
What side effects are you having with Avodart? Do you expect them to
go away after quitting the drug? Someone here suggested that some of
the side effects might be permanent.

If I read this right, you are saying that your prostate went from 110 grams
to below 80 in three months. Is this correct? I thought that the process
of reducing the size of the prostate by using these drugs would be over a
much longer period of time.
Also, what were the undesirable side effects you mentioned.

Prostate "regrowing" after surgery kind of confuses me. All
indications from this Board are that this will occur (over time). I'm
still 100% confused about how fast and to what extent this regrowth
occurs (and whether it is related to nature/type of surgery, age of
patient, etc.).
I'm the poor sap whose uro first put me under a general anasthesia
BEFORE figuring out my prostate was a bit too large --try 158 grams--
for PVP surgery. Would have been nice if he had first done an ultra-
sound for size instead of me going through this wasted time/stress to
my system, pain, night in the hospital, etc.
I then was brought back for open abdomenal surgery to gut/core the
Prostate--not TURP-- and spent three more nights in the hospital,
system further compromised by the anasthesia/breathing intubation,
etc. Finally after another harrowing week after discharge from hosp
(with catheter back in) waiting for indications of success I FINALLY
was able to urinate (as everyone said I would) very well as to volume
and force. So far bladder is emptying almost 4 weeks after surgery
(next visit to uro is on October 12th). Of course I've been coughing
since my surgery from something I caught in the hospital.
OK that was a big digression. I asked the uro on my last bladder
voiding test visit (10 days ago) about whether the remnants of my 63
year old formerly 158 gram Prostate would regrow. He lloked at me
quizically and said "Yes you might have to come back in about 30
years"(quote-unquote). I hope to come anywhere in 30 years BTW!!!
However he then suggested I take the uraxatrol he prescribed for me
(an alpha-blocker like Avodart and Proscar) to "help me pee more
easily". As stated i'm urinating decently post-op (so far). If it
won't quickly regrow in y case, why the suggestion to take uraxatrol??
I will try to nail him down further on this (to me) confusing issue,
but I wanted to put my feedback/experience into this Thread.
Summary: My uro seemed to be saying yes it will regrow, but no not in
my (probable actuarial) lifetime. But that answer (if correct in my
case) MIGHT be different if PVP is the method (instead of my very old-
fashioned open surgery) to "shrink" it..

Har... I have nowhere the experience you have, Michael!!
What I have been told is that open prostatectomy has the lowest
incidence of needing a second operation, of all the procedures. My uro
told me around 15% of TURP patients need another TURP in 5-15 years
(take the statistic for what it's worth) so for Open it would be a
fair bit lower.
Now that you've had the pain, you can enjoy the gain!
Does the uroxatral give you side effects?
If it was me, I would ask the uro if it is reasonable to not take the
drug but do a PSA and have a TRUS (transurethral ultrasound) every
couple years or so to measure your prostate size. That should tell if
it is growing.. and then you could take the Proscar or uroxatral at
that time if the pesky gland is once again getting big.
Who knows what the surgical techniques or meds will be 10 years from
now--probably better than now. You most likely have at least 10 clear
years ahead of you. So if you can postpone decisions, that might be
better.
Just a suggestion... something to ask the uro.
Back to the original point and RVer Don's response, I too think it
takes longer than 3 months to shrink the prostate by that much.

didn't even have the good side effect, that which is supposed to grow
hair, as its cousin Avacor is supposed to do.
I also was on Flomax, which side effect is dizziness. I got dizzy
whenever getting up from sitting, or crouching down. I figured that the
side effect was more dangerous than it was worth, after almost passing
out while working on a roof.
