Jerry: Congratulations on your outcome. Tomorrow (Tuesday) I am
scheduled for GL PVP to be done by a top uro (Terrance Malloy) here
in Philly. I am the OP on several Threads with the AUR acronym on
this Board. The folks have been wonderful on this Board and your story
will somewhat calm my fears.
My major concern is that the uro did not do a biopsy FIRST (I have
high PSA previous biopsies showed "only" BPH) and will do one before
surgery tomorrow (but will not know the biopsy results until several
days AFTER the surgery). If it comes back positive that's two
procedures instead of just one. I thought that was strange.
Second I'm still not clear what happens regarding requirement of
urinating on my own after the surgery as far as Discharge criteria.
I've had a catheter in for most of the past 3 1/2 weeks, it's still
in. They are keeping me overnight but even by the next morning I can't
see being able to urinate easily (if at all). Then what? Keep me in
til I do? Put catheter back in and keep coming back until I do?
On a scale of 1 to 10 how painful was everything 12 hours after? 24
hours after? 48 hours after? One week after? (10 being most
painful) n

Since you are staying overnight I will wager you will be peeing freely
by the next morning. They may well go without the catheter to start
with to see how you are doing.
No pain. After PVP surgery I had to keep reminding myself not to
overexert. However I did have a catheter overnight. As I said before
my flow was not all that great the first day but steadlly improved
from there on.
TUMT was a hell of a lot more painful. The worst part was when the
Dr. injected some kind of pain killer into the prostate.
The rest of it rather boring. I told the gal who was running the
machine that in Arkansas we would be legally married. Didn't faze her
much. She had performed 500 of them.
Will be watching to see how you do. Wednesday I take my wife in for a
Rotator Cuff operation. That will take a lot longer to heal.

Michael-
While most GL PVP are done as a day procedure, I also
stayed over nite due to other non-related urology medical conditions
I have so they could observe me- The biopsy is routinely done on
everyone who has a GL PVP AS part of the GL PVP procedure-It is sent
for examination & the results are available within a few days- It
really is not a second procedure since they are already in the
prostate with the GL lazer probe , they take a piece of the prostate
tissue during the lazer procedure for the biopsy from the lazer probe,
before they "burn off' the rest of the prostate tissue causing the
BPH. It is an easy way to find out your prostate tissue results.It is
rarely done separately before a GL PVP as its considered
unnecessarrily invasive when it can be done as part of the GL- Also
your surgeon is doing the PVP because he has determined you have BPH &
that GL will fix that so, a seperate pre GL biopsy (in spit eof higher
PSA but still BPH) is just not called for-
The GL will literally burn off all of your prostate tissue- It will
leave a hollow prostate with no blockage to prevent urination- Ihad a
cathether put in as part of the GL & they removed it about 4-6 hours
later- I was able pee painfully right away & don't get scared but your
urine will be red from the blood in the prostate from the GL & after a
few days it gets less red- Yes, until you pee on your own they will
not discharge you but as I said there will be no enlarged prostate
tissue blocking your ability to pee, so peeing should not be a
problem-
Pain after surgery was 4 on a scale of 1-10 but only when I peed- wHEN
i DIDN'T PEE, PAIN WAS 1-
Pain was on a scale of 3-4 for about 2-3 weeks when I peed (it felt
more like a burning sensation) and 0 when not peeing after 2 days-
Peeing contest accepted anytime, anywhere --:}- Good luck & the GL
should make you much better & relieve many of your BPH symptoms

Hi Jerry, Thanks for the offer to share your experience.
I am at the point where I need something done and have found a Dr who does
the PVP.
My questions are
a) since they hollow out the prostate, does that mean that if there was any
cancer not found by the random biopsy samples, there is a good chance it may
have been removed with the PVP?
b) What is left of your ability to have an erection and feel a climax. I
assume no ejaculation.
c) Is there any risk of incontinence? Do you lose control for a while?
