PROSTATE PROBLEM

I AM A RUNNER.  I AM 52 YEARS OLD .THE LAST 7 YEARS I HAVE TRAINED FOR
AN ANNUAL FALL MARATHON. DURING PRACTICE RUNS OF ABOUT 10 MILES OR
MORE I HAVE TROUBLE URINATING. I HAVE THE URGE TO GO!! BUT IT WILL
ONLY DRIBBLE OUT..SOMETIMES NOTHING AT ALL, BUT I STILL HAVE THE
FEELING I HAVE TO GO. AFTER MY RUN IT MAY TAKE A COUPLE OF HOURS
BEFORE I CAN URINATE FREELY. MY DOCTOR HAS PRESCRIBED ADVODART AND I
HAVE BEEN TAKING ONE A DAY FOR 2 WEEKS NOW. ANYONE HAVE ANOTHER
ALTERNATIVE OR HAS ANYONE EVER HEARD OF RUNNING CAUSING THIS PROBLEM?
OBVIOUSLY, THE STRESS OF DISTANCE RUNNING IS THE CAUSE BUT I'M LOOKING
FOR SOME INPUT! THANKYOU!!

nala...my syntax, grammar, vocabulary and spelling are second to none.  I do
not consider it necessary to always put a question mark after asking a
question (I use them sometimes), since it is implied that it is a question -
but that is way above your head, since anyone that types in caps is a
complete a.shole, and would be thrown out of most newsgroups immediately for
continually doing that.  Typing in caps is an absolute disgrace, and right
on top of the bad etiquette list for newsgroups.  BTW, did you notice how
difficult it is to read your stupid a.s messages typed in caps.  The rest of
the people in this group are obviously too ignorant (or too chicken sh.t) to
agree with me on this.

As far as my foul language, I call them the way I see them.  I used to be a
regular in here, before the group became a bunch of boring, redundant
bullshit about PVP's and BPH, and then I took some sh.t from one of the
participants, and now I am out.  And shame on you Derek for badmouthing me.
I used to think you were an okay person.

Anyway nala, you really ought to stop typing in caps (I was actually trying
to help you at first).  If you can afford a computer, and figure out how to
get into this ng, then you can sure as hell learn how to use the shift key,
and stop "blinding" everyone with your stupid a.s caps, because you are too
lazy to use the shift key.  If you keep doing it in other ng's, I guarantee
you they will blast the hell out of you.

Duh!!! on you DP.  I meant exactly what I said.  What planet do you live on,
not to understand what I said.  People like nala use the caps lock key, and
type everything in caps, because they are lazy, and do not want to take the
time to use the shift key to create a capital letter at the beginning of a
sentence.  Duh again, for me having to explain that to you.  I meant nala is
too lazy to use the shift key, just like I said.

And furthermore, you are also an a.shole for top posting which is another
very bad thing to do in ng's.  You answered my last post at the top and you
should have answered it below my post (it's called bottom posting for your
info).  Obviously you know nothing about this, or you wouldn't be doing it.
If you notice I answered your post after it (in the middle of the text), but
at least I followed your answer, so it can be read in the proper sequence.
You are a real jerk.  You not only know nothing about BPH, you don't know
anything about ng etiquette either.

 Pete,
       You are right. I didn't know about using the shift key to create a
capital letter at the begining of a sentence. I apoligize. I learned
something to day that I should of already known but I know about bottom
posting, just messed up, sorry. However I don't think that everybody that
makes a mistake is a jerk or any other name that you want to call them. Name
calling has no place on the newsgroup. You write a nice informative post but

Thanks DP...I really am a good person, and always try to help people, but a
couple of posters in here got on my nerves a while back and I dropped out

can start over and contribute to the benefit of the members of this

Thanks Derek...see my last post to DP.  I will try to stay in "read only"
mode unless I see something that may be wrong or misleading to someone (in
the interest of helping them), and I will also try to improve on my
vulgarity - but if it is warranted (ie, someone starts on me), I will defend
myself.  You have to admit that typing in caps is the scourge of ng's, and
nala doesn't even realize that it is very painstaking to try and read
someone's post that is all in caps (it is difficult at best, and actually
puts a strain on your eyes).

Take care Derek.  I really used to like most of you guys (and felt close to
some of you), and the group peaked out when Dr. Sancha left to go on to
bigger and better things.  He was an awful nice person (and so
knowledgeable), like I have said so many times in the past.

I don't think that any doctor can really have the time to post regularly on
NG's. One or two did post here years ago when I first started reading it. As
with Dr Sancha they probably found it too demanding of their time.

I agree, and don't see how he lasted as long as he did (and I still believe
he takes private e-mails).  As I stated many times - I wish he was my
urologist, and that all urologists were as kind, and caring, and as

I use Avodart with great benefits, sleeping longer without getting up to
pee more than once or twice a night. I also jog, though much less than
you, one hour at at time, just over 5 miles. From my personal experience
running has no effect on my BPH. Being hydrated is good advice in any
athletic endeavor. Ask your doctor how long it takes for Avodart to work
and whether there is another medicine you can take.

Thank you for your input. My doctor told me it would take a few weeks
before I see any results. i am also taking the advice about staying
hydrated. Pete, if you notice, I am taking your advice about capital
letters. i am far from lazy..as evidenced by the fact that I run 10
miles a day..have a career..and a large family. I am simply a bad
typist. no need to use bad language to make a point. I didn't realize
using capital letters was a no -no .i will try and change my ways.

I had an appointment with my doctor today. He explained to me that
there is a dramatic flow of blood to the area around the prostate
during extreme or extended physical activity. the expanding blood
vessels coupled with an already enlarged prostate restrict the urine
flow. About an hour or two after resting the flow is about back to
normal. he said the advodart should start to help after 3 weeks or so
of use. he did say that some individuals respond to flomax somewhat
quicker. I thought i would pass on this info to help anyone else
worried.

nala...did your doctor (I assume your primary care physician based on your
OP) give you an idea how enlarged your prostate is.  I assume he did a
standard digital exam.  It may take longer than 3 weeks for the avodart to
start to help (if it is going to help), and you may want to try taking the
flowmax with it (ie concurrently).  The flowmax should start working very
soon and will relax the muscles in way of the prostate.  If you take both,
one of the bad parts, is that you now may have two different types of side
effects (but you didn't indicate you were having any problems with the
avodart).  I am sure you have read in this ng about some of the negative
experiences with flowmax.  I can't take any of the alpha blockers because
they plug my nose up immediately.

If you don't get any improvement with the avodart (problem is it could take
a long time to find out), then you may want to ask your doctor about taking
the flowmax with it.  Another problem with taking two or more drugs at the
same time, is that you might not know which one is helping (or hurting) the
most, unless you phase them in and out somehow.  In this case if you add the
flowmax and you get almost instant relief, then you have to decide whether
it is worth taking the avodart for a long time to see if it decreases the
size of your prostate [which your doctor may not be able to tell (at least
not accurately) by doing a digital exam].  Sorry, I am starting to ramble.

           I have been taking Avodart for about a year and at about three
months my night time visits went from four or five times a night to not more
than once sometimes none it lasted for a couple months and I thought this is
great but after about two months my night trips to the bathroom reverted
back to the four or five times a night and has stayed that way since. I also
take Hytrin two mg in the morning. I take it for blood pressure as much as
for the prostate. I saw my Uro about a year ago  then he said it wouldn't
help the nighttime visits but for some reason something did for two to three
months. I 'm going to ask him about this but I was wondering if any one in
the group has any thoughts on this. Thanks for any input on this.

Dean...I have reread some of your previous posts in the 6/22/07 thread.  Out
of curiosity, when did you start taking the hytrin (ie before or after the
avodart, I assume before), and if you took it before, did it seem to help
your frequency (it wouldn't seem so if you were going four of five times a
night when you started the avodart, unless you used to go even more often
before that).  In other words do you have any feel as to whether the hytrin
is helping your urinary problems (as an aside, I believe the alpha blockers
are kind of passé now for treating high blood pressure in itself - but if
you have been taking them for a long time, and you and your doctor are
pleased, then forget my comment).

You will have to ask the man upstairs as to why you were getting relief for
two or three months (after being on the avodart approx. three months), and
then it stopped.  A common sense answer may be that whatever was helping,
stopped working - dunno.  I am also curious why your uro said the avodart
wouldn't help the nighttime visits, although I believe you said he put you
on it because your prostate grew back considerably after your PVP.  I
acknowledge what you said about the trabeculation also.  Did your current

That is what this newsgroup is all about I'll try and answer your nice
reply. I have been taking Hytrin for ten years or longer. I started taking
it primaryly for BPH. I think it helped some but not alot but I found that
it also helped my blood pressure.
  I wish that I could figure out what gave me the relief those two to three
months it was almost like being Heaven sent ,I think you know what I mean
but it disapeared and hasn't returned. One thing I forgot to mention in my
previos post was that my volumne has decreased from maybe 250 cc to about
150cc. I don't know why he said it wouldn't help the nighttime visits but
come September that will be one of my many questions to him.  He didn't do
the PVP he is strickly a Turp guy I had to travel to Ashland, KY  to get it
done as I am from WV and no one does it there. My prostate has always grew
at a tremendous rate, when I had the PVP 4 years ago it was 93 grams and I
assume it would be alot larger now. He hasn't mentioned have another
cystoscopy maybe because I passed on his Turp and went for the PVP.

Thanks Dean...I wish I could help you more.  I live in western Maryland and
no one (all six uro's are in one complex) does PVP here either.  I  have had
TUIP in 1995 in another county, and TURP here in 2005.  I requested another
cysto last year by one of the other uro's in the partnership (they let me do
that, which is unusual) and I could clearly see the guy who did the TURP cut
away nicely on the lateral lobes but he didn't cut the median lobe.  The uro
who did the cysto noticed it immediately (he called it a hump), and he
didn't have to tell me, because I saw it the same time he did.  He said it
wasn't bad when I asked him, but I disagree.  I had previously asked the uro
who did the TURP if he cut around in a 360 degree circle (if you follow me),
and he said yes, and that he connected the dots from the previous TUIP
slices that had scarred over and created a blockage.  I think he connected
the dots, but forgot to cut between the 4 o'clock and 8 o'clock dots (which
is approx. where the previous slices from the TUIP were).  Damn it :-) .

I will probably need another procedure again someday, and would like to try
the PVP next time.  I notice there is a guy across the border in PA who has
just showed up on Laserscope's doctor list.  I only have one person to help
me, and it is difficult at best, when I need to go under anesthesia for
surgical procedures, or my EGD's, or a colonoscopy, etc.  And having to
travel to another city just makes it worse.

The thing that bothered me the worst about the TURP was the catheter for 3
days (I couldn't stand it), which I have stated in here before (when I had
my TUIP, I had no catheter).  Are you totally against having a TURP.  The
rule of thumb is that it is more invasive, but perhaps it may last longer,
as has been stated in here many times by others.  It also depends on the
skill of the surgeon, and hopefully he will end up with a nice round channel
when he's done - lol .

Sorry for the rambling again.  Did your uro refer you to the guy in KY who
did the PVP, or perhaps you didn't need a referral.  Is your uro disgruntled
because you had the PVP in lieu of the TURP.  If you are still seeing him,
at least he hasn't dismissed you :-) .  Take care and I wish you well.

        From reading most of your old post I know you haven't had a very
easy time down threw the years and I hope things go better in the future.
  I have also spent alot of time with a catheter in me. I went on a
vacation a few years ago and ended up with a chatheter in me all the time I
was on vacation. Took it out myself when we got back from vacation. I had a
chatheter four times due to complete urinary retention.That is when I got
serious about the PVP.

 I'm not totaly against having a turp but I don't think I need any surgery
right now to bad you can't do these surgeries once and have it over with. I
located my surgeon on Laserscope website and went straight to him.(Dr.
Stehpen Edge) I think he had only done about six PVP's when he done mine and
he might not of been aggresive enough. It dosen't seem like my regular URO
cares about passing up his Turp but who know what is on their minds.

Take a look at the AUA site for discussion of the various treatments and
their outcomes...lots of good information.  TURP is the transurethral
rescection of the prostate using a cutting wire to remove tissue and the
PVP is the greenlight Photoselective Vaporization of the Prostate.  Just
do a google search on them for further information.

Some doctors say Advodart is now good.
They usully recommend FLOMAX which has its own set of side effects.
I hear that it works very well for these problems though...

I had been using both Avodart and Flomax at the same time for about two
years. Avodart does not seem to be doing anything, including one of the
side effects which was that it was supposed to help revert male pattern
baldness (like Avacore). That doesn't work either. :-)
I still take Avodart, but I quit the Flomax. It did help somewhat, but
the side effect of dizziness made me stop for safety reasons. Besides
getting up from bed or sitting, kneeling or crouching down for a few
minutes, and then getting up, almost came to passing out, where I would
have to hold onto something until it passed.
I would think that a runner, with increased blood flow to the legs,
should not use Flomax.
