TUMT vs. PVP

I have two Uros.  One, wants me to have a TUMT. My symptoms are:
1. Up at least once per night
2.  Trace amount of leaking during the day (This one bugs me the most)
3.  Usually need to urinate every hour or so.
4.  Weak and sometimes dribbling stream.
5.  Slow to start urinating, even with a full bladder

Pretty much the typical symptoms (except for the leak).  From my
research, it I'm pretty certain that PVP is the way to go.  The other
Uro (Dr. David Laub of Santa Barbara) does the PVP's and has a lot of
experience.

I guess I need some encouragement to get this overwith.  I'm only 55.  
My dear wife died of cancer about 2 years ago, so I'm on my own here.  
I've enjoyed the posting, but I'd like to hear from guys who have had
the PVP recently and their experience.

I tried the TUMT first, about 2 years ago.  My Dr. did both TUMT and PVP
but suggested trying it first.   Part of the suggestion is most
certainly financial.  TUMT being done in the Doctors office using their
equipment.

Had PVP last May.

TUMT was a lot more painful, being wide awake for the whole session with
what seemed a minimum of sedation.  They claimed that 85% got relief.  I
think it is more in the 50% range.

I question how much relief as well.  It did help a bit but not much.

But of course any time you go to a Hospital you are putting yourself at
more risk.  Especially when they use a general anesthesia.  Anesthesia
is a lot safer than it used to be.

Anyway, the PVP did the job but of course you must expect Retro.

If that worries you, you might try the TUMT first as you can always do
the PVP.  Perhaps Laub does both as my Uro did.

I had TUMT 5 years ago and I've had no problems since.  I was getting up
every hour and a half during the night and my stream was extremely weak, my
bladder wasn't emptying but the stream would totally stop.  It also took a
while to get the stream started.  Since the procedure I've had no problems
going at night.  Sometimes I get up once and my bladder empties like it
should.  Occasional retro, but not an every time thing.

I had TUMT 7/2/04 and PVP 2/23/05.  The TUMT failed to clear the half of
the prostatic urethra furtherist from the bladder and thus the PVP.  Had
the TUMT done its job, it would have been preferable since there is less
likelihood of retro from TUMT.  Although I was told ahead of time that
the PVP would be a "bladder neck sparing procedure" and only a 20%
chance of retro, the URO put one cut in the bladder neck at the 5
o'clock position.  Thus I have 100% retro.  The recovery from the PVP
was easier than the recovery from the TUMT.  The results from the PVP
were more immediate after the first 24 hrs while the results from the
TUMT don't really show up for 1-2 months.  Knowing what I know now, I
would likely have skipped the TUMT and gone directly to the PVP even
though I have the retro.  Age 62 with bph.  Uroxatrol and Flomax were
not good answers due to the tiredness and low backache that they gave me.

Had the PVP about a year ago.  Uro said to expect retro, but I haven't
had that at all (probably best to expect it though, and count it as a
bonus if you don't).  Leaking stopped, powerful stream, instant start
& stop, quick recovery, but expect three months for complete healing.

Mike,

I had PVP in April 2005 with symptoms similar to yours, though no leaking. I
was 53 at the time. Although it took 12 weeks to fully recover and see the
maximum benifits I feel the operation was very successful. Now I have almost
forgotten how bad my flow used to be even with .4mg Flomax a day. I believe
the procedure has improved since I had it. I can recommend it without
reservation.

However, choose your surgeon carefully. By that I mean find one with a good
reputation for this procedure.
