I had TUMT 7/2/04 and PVP 2/23/05. Supposedly the PVP was to be done as
"a bladder neck sparing procedure" as per the cystoscopy 3 weeks prior
to the PVP. When the procedure was done, the URO made a "single
incision through the bladder neck at the 5 o'clock position, which
opened it up nicely." Although he told me before the PVP that there was
a 20% chance of retro, I have had 100% retro ever since the PVP. The
thing that I miss most (now at 100 weeks post PVP) is the pumping
sensation of ejaculation that I used to feel prior to the PVP. Now that
the semen is ejaculated into the bladder, the pumping sensation is very
definitely diminished. Sex is still worth the effort, but I have had to
get used to some new sensations. (I'm 62) If you have further

Ah, that's what I feared. The main physical sensation for me at orgasm
is the fluids being forced through the sphincter on the outlet of the
prostate (at least that's what subjectively seems to be happening).
It's not the only thing that makes the orgasm satisfying, of course,
but I would miss that.
I entirely agree with Dave's point about the major importance of the
'way there' as well as the end of the journey, but if you've been used
to ending up in Las Vegas and suddenly you're only getting to
Middletown ... well you might feel a bit like you're missing out.
I guess I should give the pills another try first. I tried Indoramin a
few years ago and it was just awful. Maybe there are better options,
now.

I have been on Uroxatrol and Flomax. After a period of time on the
Uroxatrol, I experienced tiredness, grumpiness, depression (?), and
some low back pain. If the TUMT would have solved the problem, it would
have been the way to go with my bph, but it failed to clear half of the
prostatic urethra furtherist from the bladder. Thus the PVP. All in
all, rather than be on the pills, I am happy with the outcome even with
the retro. I learned early that I did not want to be on the medications
indefinitely! My understanding is that the risk of retro is directly
proportional to the amount of surgical intervention at the bladder neck.
