medication or surgery

I've been on UraXatral for the last month since being diagnosed with
BPH; starting at 10 mg. with dinner, doubled about a week ago to 20 mg.
with breakfast and with dinner. I had a cystoscopy and bladder scan a
month ago. I've seen some mild improvement, but yesterday was a hard
day.

While driving to the airport (90 minutes away), I had to stop for the
bathroom 4x, for a total of 12x throughout the day. A few of those
times, I had leakage to the extent that I could feel droplets on my
legs. My boxers were damp more than once and I truly felt I wouldn't
make it to the bathroom. Fear of arrest prevented me from relieving
myself between cars in a parking lot.

My current thinking is to continue the UroXatral and, next time I see
the urologist, ask for addition of Proscar in hopes of shrinking my
prostate medically. Failing that, from what I've read, I think that if
I were to choose a surgical procedure (from what I know now), it would
probably be PVP.

My major question is how long should I give the meds to work without
adequate improvement before giving up hope in them and moving to the
next step?

I'll be 48 this fall.

A couple of other questions: Any side-effects from Proscar? And any
side-effects from PVP, and how long to recovery?

Stop torturing yourself and get the PVP if your doctor recommends it, you
will be grateful you did.  The majority of contributors here (including me)
had successful and relatively easy experiences and no longer define our
lives by how far away we are from a bathroom.  Jim W.

Hey Mountainguy, I agree with Jim.  I had mine 18 months ago, and I
don't know why I waited.  Stop the torture and find a uro that has done
150 PVPs.  Good luck, Joe

Same story as the others.  Go for the PVP.  Post your location and
someone may be able to recommend a good doctor if you are looking.
BTW:  The only downside is that retro will most likely result but it is
not a big deal.  Not being able to pee is!!  I am not so certain but
seems to me that Flomax and Uroxatral made incontinence worse for me.

Ditto on the PVP.  The meds all have side effects, and all have limited
effectiveness.  The PVP will more than likely give you complete relief
(I NEVER peed like this, even when I WAS a teenager), with no chemical
fog.  Retro is once or twice a week - dribbling is all day every day.
You could spend another ten years of increasing misery and THEN get the
PVP, or just get it over with and get your life back now

You're going to have surgery sooner or later, so I would suggest doing
it now because of the following.  Your bladder is being stretched now
by your inability to completely empty it.  The longer this goes on, the
less likely it will return to normal once you have the surgery.  If you
wait too long, you may not be able to completely empty your bladder
even with surgery.  So I would suggest not waiting.

Talk to your doc. Maybe a different med will work better. Don't rush
into PVP but if meds don't work, PVP has produced good results for
many.

Since you're talking primarily about an irritative symptom see:

under the heading "Alpha Blockers in Combination With Antimuscarinic
Agents". The antimuscarinics will stop the urge and the leakage at the
risk of reduced throughput. Apart from the urge described above you
don't mention anything about other symptoms such as retention.

I presume you've been told that you have a large prostate. If so
shrinking via Finasteride (Proscar) may take 6-12 months. See:

Consider Proscar or Avodart.  These block the hormones which keep the
prostate enlarged.  They can have side effects also.
