I am now 55 years old and read some articles related to the prostate gland.
I was wondering if urologists can easily determine whether or not a patient
has an enlarged prostate gland? If so, please post information about how
the test is done. Is it different than the rectal test that a GP does
during a yearly "physical" exam? Can a GP doctor do the same test or
should I ask my doctor to refer me to a urologist?

Hi Jason...

The UROs have at least 3 ways to establish the size of the prostate...1)
 the Digital Rectal Exam; 2) the TRUS (Trans Rectal Ultrasound); 3) the
Cystoscopy exam.  One thing to keep in mind is that bph can block the
prostatic urethra in different size prostates.  In my case, my urethra
was blocked in a 35 grm prostate.  In other cases, the blockage may
progress over the years until it becomes 150-200 grms. A lot depends on
how much a person is willing to put up with.  Some treat it with
watchful waiting, with medications or surgical interventions.  Some UROs
do not want to do PVPs or TURPs on prostates over 80-100 grms.  The
longer one waits, the higher the possibility of complications. In my
brothers case, he waited until his was 105 grms and had to have an open
prostatectomy.  It was not a fun experience.  He could have found a URO
who did PVPs on his size prostate but did not want to go to the trouble.

I would be very careful about getting a referral to a URO.  Many times,
the referral may be to a URO who only does TURPS.  Do your homework and
find out if there are UROs in your area who do PVPs. Check out the
Laserscope.com site for your area.  A good place to start.  We all make
the best choices with the information that we have.  Good luck!

Hello,
Thanks for your post. I'll keep your advice in mind when I develop
prostate problems. As far as I know, I don't have prostate problems at
this point in
time. However, I will have my doctor (GP) do a Digital Rectal Exam during the
next visit which is next month. I will make a note that the PVP is superior to
the TURP. I've already learned some things that were not mentioned in the
article that I read about the prostate gland.

Jason...welcome to our group.  I notice you have meandered over from the
sci.med ng.  Did you read my recent post to Khan in that ng (he has an
interesting uro problem).

Chockman's response to you was excellent.  This is good ng and there are a
lot of people in here with valuable experience.  Most of the discussions
center around PVP.  I have had a TUIP and TURP myself, and I would
definitely recommend the PVP even though I have no personal experience with
it.

Are you starting to have the typical mid fifties urinary problems, or are
you just curious (you didn't say).  Different uro's (and GP's) will estimate
prostate size differently (using the digital method - what you called the
rectal test).  Personally I would not go to a uro unless you were having
problems.

BTW, we have an awfully nice (and very knowledgeable) doctor in the group
(Dr. Sancha)from Madrid, who specializes in PVP's, and teaches around the
world.  You may want to search his posts.  Just search "fgomsan" in the
"from" block.

Pete,
Thanks for your post. I do believe that you were the person that referred
someone else to this newsgroup. I decided to take the advice that you
meant for someone else. I don't believe that I have prostate problems but
wanted to know what to expect when I do develop prostate problems.  I have
kidney problems and have some problems related to peripheral edema. I am
not on dialysis and will never be on dialysis. I read a book written by a
doctor that is a kidney specialist. I am following a special diet and it's
helping. Thanks for your kindness. I have noticed that some people in
these newsgroups are not kind and seem to discourage anyone but themselves
from providing advice. I call them net cops or newsgroup cops.
