Cystoscopy & Possible Urethral dilation for slow flow and increased frequency at night

62 yr old who had TUMT 7/2/04 and PVP 2/23/05.  Started having symptoms
again about Christmas time (more frequent night awakening and straining
to start the flow.)   Went in to the URO today for cystoscopy and
possible dilation.

The URO showed me via monitor that the channel through the prostatic
urethra and into the bladder is very open.  He sees no reason for any
kind of further intervention. There was no need for dilation. While the
cysto was still in place, he injected saline or ? into the bladder until
I had a full bladder.  He then did another flow test but I failed to get
the results from the flow test. Last weeks flow test showed a max of
10.6 ml/sec.  I requested a written copy today's  flow test and his
cystoscopy report; they should come in a week or 10 days.  His statement
to me was  that 95% of men who have had TUMT and PVP get very direct
relief.  The other 5 percent (of which I am one) may have continuing
problems with "overactive bladder."  He wanted to know if I had ever
tried any of the sleeping medications.  My response was that I was
reluctant because most would disqualify me from the privileges of my
private pilot license..  I recently purchased some Valerian root along
with Melatonin and I am going to give that a try for helping with the
sleep.  His other suggestion was to try another of the 6 or so meds that
are on the market for "overactive bladder."  I had tried Detrol LA but
did not notice much difference in nighttime patterns.  So I am going to
try a month on Enablex 15 mg which he wants me to take with my supper.
See him again in a month.

Hi Chockman,

I can understand that an overactive bladder might cause more night
trips.  But why would it cause "straining to start the flow?"  DId he
explain this or do you understand it?

Hi Bruce...

My experience is that it is relatively easy to start the flow with a
full but not overfull bladder.  I find it difficult to start when the
bladder is overfull. The overactive bladder is sending a mild signal of
the need to go.  The choice at night is to try to ignore the signal and
thus have interrupted sleep or get up and empty the bladder.  It seems
take more effort to start voiding on a partially filled bladder.  Does
anyone else experience this?  An analogy is like having your car's idiot
light starting to flicker off and on.  That's how my bladder feels.  It
will be interesting to see if the Enablex helps.

Absolutely Chockman...I have stated over and over (last in my kidney post)
that I can get a good flow during the day, but not during the night when my
bladder is not full, but I feel the urge.  I will not repeat everything.  I
have written at length about this...Pete
