pvp and urethral stricture

How frequent is urethral stricture after pvp? And can cystoscopy cause
stricture?

Jesse,

Any urethral instrumentation can cause a urethral stricture. the
chances are higher if the instrument has a bigger caliber, stays longer
in the urethra or if it is manipulated vigorously instead of carefully.
PVP caused a 7% stricture rate in a study from Germany, but they used a
26 F resectoscope similar to what urologists use for TURP. I use a 23 F
scope, and I have not seen a urethral stricture causing bothersome
symptoms yet after more than 350 patients.

Dr. Sancha...Is the main reason that urethral instrumentation can cause a
stricture, due to an inflammatory response.  If so does it usually go away,
or can it remain resident and keep aggravating itself so to speak.  And can
anti-inflammatory drugs (either Rx, or OTC) get rid of it to the point where
it was before the procedure.

Also, are corticosteroids (eg, the dreaded prednisone) ever used to treat
chronic non bacterial prostatitis.  I don't believe American doctors use it
(I have asked), since it is not in their training.  I don't see why not,
since it's used for everything else (eg, pulmonary problems, colitis,
proctitis, post surgical inflammation, etc)...Pete
