In recent years with a 70+ gram prostate my PSA varied between 7.5 and 9.0. On my first PSA since my PVP in July it now 5.1. Is this what one should expect? I had expected the lack of prostate tissue to give lower reading. Derek. Derek: how large is your prostate now? You indicate "lack of prostate tissue", but maybe it is not that much that was removed? My uro who did my PVP told me PVP takes out a lot less tissue than a TURP would have, but did not give me any specifics. Maybe this statement will relate to your situation? Just a wild guess. I commented once before that you must have had "PVP Light" There is no reason not to take as much tissue with a PVP than with a TURP. He did not just bore a new channel for me, he took a lot of tissue as both lateral lobes and the median lobe were pressing on my bladder. Derek. > Derek: how large is your prostate now? Derek, one should expect PSA to be 30% less after PVP. That is your baseline PSA now, you should check now if there is progressive rise in succesive determinations. Tissue removal on PVP for a 70 gram prostate can be as much as with TURP, it depends on the surgeon's technique and the time spent in the procedure. Some surgeons think it is not necessary to remove more than 30% of the gland volume to get a good clinical result. I personally think PVP can obtain exactly the same tissue removal as TURP, it just takes a little bit longer. I take no medicine and my first PSA is 1.9 after PVP. Unfortunately, one of my good friends got positive biopsy weeks ago after his recent PSA test of 5.2 ( his last before this was 4.3). He is planning to have image-guided rediation therapy (IGRT) soon.
