Tom Harms - 14 Oct 2005 22:59 GMT Home now with catheter for about 4 or 5 days. I had a lot of blood (surgeon thinks mostly from bladder). I stayed an extra day to irrigate the bladder, but was unable to get enough blood out to come home without a catheter. I'll know more after my Tuesday appointment with the Doctor. As a reminder, I had a very big problem with Post Void Retention of 1,000 ccs. Tom (age 62)
Tom Harms - 18 Oct 2005 18:48 GMT Home now (18 Oct) with catheter removed. I'm basically a slow leak or constant drip, but no visible blood. So far, I can not get a real stream going. I'm continuing to intake liquids hoping to get a stream when enough is in the bladder. Of course, I'm concerned about over-filling the bladder ~ but I think the slow leak will prevent that. I'll continue to be very concerned until I can get a stream going.
P.S. When the catheter was removed they inputted 360 ccs and I voided 420 ccs, but it was basically a slow leak.

My bladder scan showed only 75 ccs, so my bladder is definitely emptying. After 6 days with a catheter, Doctor thinks I need some time for my system to tighten up. Right now, I'm constantly dripping into Depends, etc.

Yes, your system need time to tighten up. Be patient. We wish you well. Did any body ever need catheter over 36 hours after surgery pvp?

Tom (age 62) Tom Harms - 19 Oct 2005 19:00 GMT Actually, the first 48 hours after surgery were Continuous Bladder Irrigation (CBI), which was not particularly pleasant. This is a system where fluids are continuously inputted and simultaneously drained out of the bladder to remove blood clots. We would go through a 2000 cc jug of fluid in about 30 minutes. The hospital supply room requested an explanation for the sudden demand in fluid. At one point, I had to be manually irrigated because blood clots were interfering with sufficiently outputting the fluids, resulting in extreme pain due to an extended bladder. I also experienced hypervigilance, which was relieved by an anal suppository. Right now, I'm comfortable at home wearing Depends and gradually getting some control back. One thing is for sure, the prostate is no longer interfering with urination and my bladder is emptying. It may take a few weeks to get this slow leak problem resolved and tighten up, but I'm already seeing improvement. Tom

Tom (age 62) Tom Harms - 19 Oct 2005 21:48 GMT One of the Doctor's theories on the excessive bleeding was: Due to high (1,000 cc Post Void Retention), many blood vessels in bladder were used to pressure from fluid retention. Once the bladder was totally emptied, the blood vessels bleed until they collapsed. Strictly a theory ~ later the Doctor thought maybe *some* of the blood was coming from the prostate. Certainly the PVP procedure altered the status quo. :-)

I found that mild exercise, such as walking around the house but avoiding stairs for the first 3 or 4 days, helped me manage my early-stage healing. I then--only because I had read this newsgroup-- avoided any "pressure" on the arterial walls--not lifting anything, even if light weight. I was cautious for the first 2-3 weeks. After that, I was lifting furniture and running up stairs like a young buck. All the best to you. Hope the end result meets your expectations.

I just ordered a batch (100) Conveen Drip Collector - 3 oz, Normal ~ when I graduate to the drip only mode ~ not there yet. Basically, my stream is clear, but there is normally a small discharge of pink/red fluid initially. My PVP was actually on 12 Oct ~ about 3 weeks ago. I have an appointment with my Doctor/Surgeon on Friday, 4 Nov. Tom

Wants me to stop the post-op Flomax, but continue the Proscar, since there is still a very small amount of blood in urine. Wants me to get back to normal exercising. I'll see her again in six weeks. I'm patient here, because I (age 62) was one liter Post Void Retention (PVR), prior to PVP on 12 Oct. From all that I've read, this will improve with time. I'm very happy that my bladder is basically empty after several years of major PVR. Maybe it will take several months to get the plumbing working correctly. Tom

I went back to bed with tee shirts to soak up the blood until I released again at the toilet. Serious red. I went into Bethesda Emergency Room this morning. Urine tests indicated a Urinary Infection and I was given an IV and no 30 day supply of: Levofloxacin (Levaquin Eq)
I'm now back to a Foley Catheter until Friday when I see my PVP Surgeon on Friday morning.
P.S. I had PVP on 12 Oct ~ 48 hours CBI and 4 days Foley. I've been totally incontinent since.

Today, the Urologist took me off the Proscar (since the bleeding has stopped) and started me on Detrol to see if that helps. My Doctor is looking into Oxytrol. In the meantime, my incontinence has really improved. I'm down to one or two Depends a day ~ primarily for comfort. I'm thinking this whole incontinence issue will resolve itself with another two or three months to settle down. I really do not want to take any meds. Anyone have experience on this situation? In answer to your question, I needed a catheter for 5 days after pvp in April this year. However, since the 5th day was a Friday my uro decided to leave it in over the weekend, thus making it 7. . The long time was due to a lot of initial bleeding from the prostate and the concern that scar tissue might develop.
The first day without the catheter was wonderful as anyone might imagine. However there was no leakage, ever. I would think that the sphincter muscles should tighten up immediately unless it has been damaged. But pvp is supposed to avoid damaging muscle tissue.
Tom, I hope your problem clears up soon. Perhaps the bladder, used to pushing hard, is still pushing and overcoming the resistance of the internal and external sphincters. I am 2 years older than you are and I also visited 5 uro between 99 and 05. Took Cardular, Flomax and Proscar for years and had pvp 9 weeks ago. Very satisfied. My prostate size was 100g with median robe. My peak flow is 35ml/s vs <<5 ml/s. Post pvp, Dr. Te asked me to take Advodart for 3-6 months but like Spread_DeMocracy said that I rather to have another pvp after years than to have this medicine's postive and negative side effects. What was you size? Do you need to take medicine to shrink the prostate?

He did not suggest anything to shrink the prostate after pvp nor did I think to ask about it at the time. He did want me to continue Flomax for several weeks after and I found it helped the flow. I tapered off around 8 weeks. After that I tried Flomax for a couple of weeks out of curiosity to see if it made any difference. It did not. Since then my peak flow is 20-25 ml/s which is fine with me. Time will tell if my bph returns. My father had a TURP when he was around my age and has been fine ever since. He is now 87.
Interesting that you saw 5 uros. I do not think that my bph condition changed much in the last six years before pvp, so I am still mystified that I got such wildly differing opinions from each uro. It certainly taught me to be active in managing my own health care.
Reminds me of the respected head of urology at Duke School of Medicine in North Carolina who showed me the x-ray they had taken and explained just how serious my cancer was. I was shocked since this was the first time I had heard this. As I looked more carefully I noticed the name on the x-ray was not mine!
