Bruce, if you are not able to get an answer from the newsgroup, check out
the PVP websites, or even contact the company if you can't find any...I did
and got wonderful cooperation from their sales rep.   He referred me to 2
doctors within 200 miles.  I picked one and interviewed HIM.  I was glad I
contacted the company since local doctors/hospitals were clueless about PVP
and only wanted to do roto-rooter...and I would still be waiting many more
months, yet, to have had even the roto rooter done.

P.S.:  Since you may drive some distance to get there let me share this....I
drove in with my wife the night before. Checked into the hospital at 10:30
AM, was in surgery at 11:30-ish, out of surgery an hour later, released from
the hospital at 3:15 and was driven right back home.   All the best to you!
