My wife was put on an 88mcg daily dose of Synthroid to correct a hypothyroid
condition. It made a tremendous difference: correcting the her problems of
fatigue, depression, feeling cold all of the time, and loss of hair.
However, it also resulted in too low a TSH. So the doctor put her on 75mcg
which has raised the TSH into an acceptable range but brought back those
symptoms, particularly the loss of hair.
Since Synthroid is not available in an intermediate strength we were
wondering if simply taking a half of each tablet as single dose (they are
scored) would be a way to test the efficacy of a small increase in strength
to help mitigate the problems without getting the TSH too low. Another
thought would be to take each tablet on alternate days so that the average
intake would be equivalent to about 81-82mcg per day.
We will be checking these thoughts with her endocrinologist but would be
interested in any experience anyone might have had with dose management in
the manners described. Or any references that might be pertinent.

With the long half-life of levothyroxine (Synthroid) and less than ideal
absorption, either option you listed would be viable if she and her
endocrinologist decided to try such a dosage adjustment.
Either way, she should be taking it on an empty stomach at the same time
every day (usually in the morning), and she should not be taking any iron
supplements, fiber supplements, or antacids within several hours (4 hours
at least, some suggest 8 hr) of the medication.
