I don't the total number of schools that offer the 3-year Pharm.D. but
these schools do:

1. University of Southern Nevada: College of Pharmacy in Henderson, NV
http://www.usn.edu/html/pharmacy_program.html

2. Midwestern University: College of Pharmacy, Glendale in Glendale,
AZ
http://www.midwestern.edu/cpg/
(they also have a campus in Chicago that offers the traditional 4-year
program)

3. University of the Pacific: Thomas J. Long School of Pharmacy in
Stockton, CA
http://www.uop.edu/pharmacy/default.asp

4. Ferris State University: College of Pharmacy, Big Rapids, MI
http://www.ferris.edu/colleges/pharmacy/index.cfm
I think you have the option to finish in 3 years + 1 semester or in 4
years here

5. Albany College of Pharmacy, Albany, NY
http://www.acp.edu/index.php

Hope that helps,

Brandon
