Anybody suggest an alternative? domperidone, metoclopramide?

There is no straight forward alternative.  If you're prescribed Zelnorm,
presumably the primary symptom is constipation, and neither domperidone
nor metoclopramide (whose pharmacology is different anyway) would be
helpful.

High dose glutamine could be worth a try if it hasn't been tried already.
Melatonin and low dose naltrexone <http://www.lowdosenaltrexone.org> are
another two things that help many people with IBS, but I don't know if
they work for /C, /D or both. Also, some antibiotic protocols have been
used for IBS with good results.
