Would your physician arbitrarily place you on a 'high(er)' dose rather than
start you on an entry level 'low(est)' and increase it gradually until an
optimal level was reached?

No
But the standard doses jump in abt 10mg increments
right?
Suppose one could jump up in one mg increments?
Or 2 mg increments
What does it HURT to start at lowest does and slowly
tweak up to exact does vs jumping in some arbitrary
increment?
