__Hospitals and physician med orders regarding administration

Hi,

Do physicians always order medications in a 'strength' like 200mg or
20mg / hour ?

Would a physician actually choose a particular infusion like 2mg/ml
and then order it as ml/hour (or whatever time unit it is).

Or is this all left to the pharmacist or the nurse?

Depends on the hospital.  Some have "standing order sets" i.e. Physician
will order Pitocin drip at x units/min.  Standing orders state that all
Pitocin drips will be x units in 1000ml.  Some physicians will just write
2mg/kg/min and leave it up to the pharmacy to make a concentration.  Most
hospitals prefer to have pre-mixed solutions with fixed concentrations, then
only the rate will vary and people aren't confused by ever changing
concentrations.

Yes , Doctor must specify the strength of any med they order as well as
dosage form and a rate if its iv otherwise the order will be invalid and
the pharmacist must contact physician to verify the order .

We have several MDs here that will write an order with Drug X  then "...give
apporpriate dosage per age/weight, contact pharmacy for correct dosage"
then we have to write the order.
