I need help  help  me with what are the optimal drug therapy/good
combination and how many agents, for uncontrolled hypertension,
especially if the patient also has diabetes and is on insulin therapy.

You can view the ADA's guidelines here:

In summary, it suggests an ACE inhibitor or ARB first, adding a thiazide
diuretic, then beta-blocker, and/or calcium channel blocker if needed.

Optimal therapy depends upon individual patient response to the meds, as
well as other patient-specific details/comorbidities.
