Does anyone have any ideas as to what should be included in a STATEMENT
OF INTENT FOR A PHARMACY PRACTICE RESIDENCY? Would anyone have any
samples of any such statments or parts of ones? Anything at all would
be of help. Thank you.

You may want to contact Kim Mowrey or Kim Mattock at the VA Tampa. Kim
1 (mowrey) is our residency program director and Kim 2 (mattock) works
with the ASHP on their residency accrediation.
