circa 100,000 US deaths/year drug-drug interactions

And that doesn't include single pharmaceutical
caused deaths (like from Vioxx.)

It seems that the med/pharm industry hasn't
much of a clue about the dangers of its products.

I get a mental picture of a demolition derby.
The dox and the pharmies say, "let's give the
patient these two (or three or nine) drugs
together and see if they crash into each other!"

"Oops, they did?  OK, better add another $11,000
to the bill.  'Corpse costs', you see."

Most likely about 0.0003 percent....  

The number who died because they DID NOT take their
prescribed medications, or took them incorrectly , is a hell of a lot
more.

Bob says that this is 0.0003 percent of the number of
people written prescriptions yearly in the US.

Now, 100,000 is 0.0003 percent of what number?
