Scents in laundry detergent can cause hives.
I'll bet certain fabric dyes can cause hives.
Fabric softener sheet scents can cause hives
Scents in liquid fabric softner can cause hives.

Sweating would make it worse, if it is a chemical
in the sheets.

And how about your blankets? Wool? Synthetic?

Good point, I had that problem with the Melalueca laundry softner.

Jake,

1) give us more info - what did you doctor believe was the cause?
2) I've had them before and they naturally became worse during the night
just because of regular changes that occur in your body chemistry during the
night.   I think the other advice about finding a nighttime allergen is a
good one, but it may be something else.
3) have you tried eliminating everything from your regular diet and routine?
stay at a friends for a few days and don't bring ANYTHING from home and tell
them to cook whatever they want.  don't bring shampoo.  don't bring anything
except the clothes on your back - and even then - wash clothes with his
laundry materials when you get there.
4) the best thing for my hives was a 24 hour Allegra pill.   consult with
your doc about that.   take it a few hours before the hives typically start
because it reaches its peak blood concentration a few hours later.
