Why don't all people in the same situation then get metabolic syndrome or
diabetics?  Because those who do also have a genetic basis for doing so,
recent research shows.  There are no simple one cause fits all effects in
humans.

It's a review article, with a limited scope. The authors only mention
metabolic syndrome in one sentence, and it was in the context of several
other conditions.

Metabolic syndrome is as you said. It is very unlikely that antioxidants
or genetics alone are the deciding factors for who develops metabolic
syndrome.
